Manager - OneCare Kansas

Wyandot BHN is seeking a dedicated Manager to join our OneCare team. The OneCare Manager is responsible for the development and supervision of the OneCare Kansas Program. Assists team members to provide exceptional service to youth and adults enrolled in a OneCare Kansas and/or the SMI Team, including development of a Health Action Plan or Treatment Plan and care coordination services.  The OneCare Kansas model of service delivery supports the provision of timely, comprehensive, high-quality OneCare Kansas services that operate under a whole person approach to care. The whole-person approach to care addresses all of the clinical and non-clinical care needs of the individual.

 
Position Requirements

Accountabilities:

  • Oversee the provision of OneCare Kansas services including care management, care coordination, health promotion, comprehensive transitional care, member and family support, and referral to community supports.
  • Recruit, interview, and hire staff for the OneCare Kansas Program/SMI case management team.
  • Provide or otherwise assure HHSP/SMI staff receive all required training.
  • Provide supervision and assure all HHSP/SMI staff meets agency performance standards.
  • Assure all OneCare Kansas members receive a minimum of at least one (1) OneCare Kansas service a month.
  • Collaborate effectively with other Wyandot BHN and community service providers.
  • Assure the HHSP meets or exceeds program outcomes.
  • Coordinate all SMI case management referrals
  • Collaborate with other programs to identify individuals who could benefit from SMI case management services
  • Complete intake assessments as needed; either in the client’s home or at a Wyandot BHN location
  • Monitor program data and develop reports to enhance program services
  • Integrate trauma informed care into daily practice and team development
  • Participates in management functions at Comprehensive Outpatient Services
  • Assure the OneCare Kansas Program provides and meets State standards for the following functional components:
  • Provide quality-driven, cost-effective, culturally appropriate, and person- and family-centered OneCare Kansas services;
  • Coordinate and provide access to high-quality health care services informed by evidence-based clinical practice guidelines;
  • Coordinate and provide access to preventive and health promotion services, including prevention of mental illness and substance use disorders;
  • Coordinate and provide access to mental health and substance abuse services;
  • Coordinate and provide access to comprehensive care management, care coordination, and transitional care across settings. Transitional care includes appropriate follow-up from inpatient to other settings, such as participation in discharge planning and facilitating transfer from a pediatric to an adult system of health care;
  • Coordinate and provide access to chronic disease management, including self-management support to individuals and their families;
  • Coordinate and provide access to individual and family supports, including referral to community, social support, and recovery services;
  • Coordinate and provide access to long-term care supports and services;
  • Develop a person-centered care plan for each individual that coordinates and integrates all of his or her clinical and non-clinical health-care related needs and services.

Qualifications:

Education: 

  • Master’s degree in the human services field, with emphasis in social work, psychology, rehabilitation or related field, preferred but education and experience will be considered. 
  • Current licensure by the KS Behavioral Sciences Regulatory Board as an LMSW, LSCSW or LCPC, preferred but not required.

Experience:

  • Team lead or supervisory experience, a plus.
  • Experience working in a hospital or primary health care setting, preferred.
  • 1 – 2 years team lead or supervisory experience, a plus.
  • Familiarity with evidence based practices for medical treatment including but not limited to diabetes and asthma.  

Skills & Abilities:

  • Ability to initiate and develop solutions in clinical, personnel, and other job related situations.
  • Ability to provide appropriate clinical and personnel supervision.
  • Ability to provide direction for programming and planning purposes.
  • Ability to work well with diverse populations
  • Excellent verbal and written communication skills.
  • Excellent ability to work collaboratively with colleagues and stakeholders.
 
Full-Time/Part-Time Full-Time  
Shift Days  
Hours Monday - Friday, 8:00 am to 5;00 pm  
Pay Range $43K without Masters degree. With a Masters degree would start at $50K
