Description

The Medical Doctor (MD) provides primary care to clients and their families within a care model that emphasizes health promotion, disease prevention, and client education, and provides primary care functions and participates in the development, implementation, monitoring and evaluation of programs and services for individuals, families and the community within the scope of practice mandated by the College of Physicians and Surgeons of Ontario. The MD functions interdependently with the other members of the Primary Care team within a health promotion framework and works in a collaborative fashion to meet program goals and objectives

Program-Related Activities

  • Performs assessment, diagnosis, treatment, health education/counselling and other medical functions as required according to the College of Physicians and Surgeons of Ontario standards of practice.
  • Provides primary care to individuals and their families by taking into account risk factors, (environmental, social, psychological and medical) in a manner which can be understood by the client, and when applicable, their families.
  • Performs clinical duties by seeing and assessing clients for routine health and illness care, prescribing treatment, during scheduled hours of service, providing on call service when scheduled and/or making home visits as necessary, and by keeping other physicians informed regarding problems arising when on call.
  • Provides outreach to clients in the Carlington client service area, as needed.
  • Contributes to the development, implementation, monitoring and evaluation of medical protocols and directives.
  • Participates in the Centre’s on call service as directed.
  • Works within a health care framework which emphasizes health promotion and education, disease prevention and recognises the special risk factors (social, physical, psychological, etc.) which may affect the health of clients.
  • Acts in an ethical and professional manner in accordance with the guidelines of the College of Physicians and Surgeons of Ontario, the Canadian Medical Association, the Canadian College of Family Practice and the policies and standards (written and unwritten) of CCHC.
  • Coordinates or is involved with patients during periods of hospitalization or with community health professionals when patients are receiving in-home health care.
  • Works with the health care team to plan and evaluate programs and develop or initiate health teaching, screening programs or workshops by identifying health needs of clients and various other community groups.
  • Provides consultation support to the Nurse Practitioners, Registered Nurses, and all other allied health care colleagues as requested.
  • Maintains and develops professional competence through appropriate continuing education methods (e.g., peer interaction, literature review, conferences, courses, staff development leave).
  • Facilitates the administration of the health centre by identifying areas where the development of protocols and procedures is required to improve client services or to promote effective staff functioning and participating in committee work and staff meetings.
  • Ensures appropriate, comprehensive treatment is delivered by maintaining complete and accurate medical records, participating in chart reviews and discussions of problem cases, making referrals, and answering patient inquiries and directing them to the appropriate treatment.
  • Participates in the development, implementation, monitoring and evaluation of treatment, education, counselling, and health promotion programs and services for individuals, families and the community.

 

 
Position Requirements

Education

  • Medical degree from an accredited university, with additional certification in Family Medicine.
  • License to practice with the College of Physicians and Surgeons of Ontario.
  • Additional training in an area relevant to the Centre’s services.

Professional Experience

  • Three to five years full-time primary care experience; experience in a community-based setting or combination of community and hospital or public health settings, an asset.
  • Experience and interest in working with marginalized and diverse clientele, such as low-income, high-needs, newcomer, and a variety of multicultural communities.

  • Thorough knowledge of and proficiency in current physical assessment and treatment methods.
  • Ability to apply health promotion concepts, strategies, and philosophy.
  • Knowledge of and interest in counselling an asset.
  • Ability to develop and implement programs using an inter-disciplinary collaborative approach.
  • Thorough knowledge and understanding of relevant community resources.
  • Excellent interpersonal and communication skills, oral and written.
  • Demonstrated ability to model “client centred” service delivery.
  • High level of organizational and time management skills in a fast-paced environment. Demonstrated ability to prioritize, to be flexible, to meet deadlines.
  • Demonstrated ability to work in a multi-cultural, multi-disciplinary team environment.
  • Demonstrated proficiency in the use of computers and various software applications, including Microsoft Office programs, client database software, and electronic record keeping in a client services setting.

Linguistic Profile

  • This position is not designated under Carlington’s French Language Services Plan.
  • Fluency in English is essential.
  • French/English bilingualism is preferred.
  • A third language relevant to our client population is an asset.

Salary Range $135.33/hour - $156.72/hour
