Job Summary: To assist the Program Director in ensuring efficient clinical and administrative operation of the Early Intervention Support Services Program. Provide clinical oversight/supervision as needed to EISS team members as well as provide clinical services to clients on assigned caseload.
Duties and Responsibilities:
Clinical Duties
Intake:
Conduct initial and ongoing assessments involving the collection, analysis, and recording of significant bio-psychosocial information. Render diagnosis at time of intake. Participate in Mobile Outreach responses for the initial evaluation and referral of consumers to the EISS program.
Clinical Services:
Provide individual, family and group therapy as clinically indicated. Maintain regular therapeutic contact with clients on assigned caseload. Develop treatment plans with clients at time of intake and review as necessary in consultation with prescriber. Provide crisis intervention.
Documentation:
Complete documentation for all services rendered in Anasazi within 24 hour of service delivery. Ensure that all documentation is up to date and accurate in accordance with agency standards, accreditation and funding source requirements.
Case Management:
Coordinate treatment with relevant staff/programs both inside and outside the agency. Responsible for overall management of caseload by ensuring that clients are referred to appropriate level of care in a timely manner.
Productivity:
Expectation is to spend 65% of clinical hours engaged in face to face contact with clients. Provide routine productivity reports to Program Director. All schedules, statistical reports/logs, and clinical reports are to be submitted within designated timeframes.
Administrative Duties
Staffing:
Assist Program Director in the recruitment, hiring and training of program staff. Provide supervision of professional staff and assigned interns as indicated. Provide feedback to assist Program Director with information to complete probationary and annual performance reviews of staff. Ensure adequate clinical program coverage during the site's operational hours. Monitor and troubleshoot daily issues from staff. Designated as Novatime proxy to assist with time sheet management of staff.
Program Development and Evaluation:
Assist in identifying and developing strategies to assist with the development of program and client needs. Participate in developing QI program and in monitoring / implementing change
strategies identified through process ( PDSA model )
Statistical Evaluation:
Coordinate preparation of routine statistics for the completion of monthly/annual reports. Gather and analyze data for QI process. Coordinate collection and assessment of monthly intake data.
Community Relations:
Assist in developing and maintaining relationships with community agencies and organizations that support EISS and its clients. Provide information to service providers and community groups through speaking engagements when needed. Represent the program/agency at external forums when needed
Productivity:
Monitor program and staff productivity levels to ensure that contractual obligations and staff targets are being met. Explore and trouble shoot barriers that are impacting productivity levels. Implement measures to address any identified barriers to productivity.
Other duties:
Participate in a rotation of on-call telephonic coverage, to ensure consumer needs are met after program hours. Utilize supervision and training as opportunities for enhancing professional competence. Performs all duties in a professional and ethical manner consistent with agency, licensing, COA, and professional standards. Other related duties as assigned.
