Purpose of Your Job Position:
Responsible for the delivery of resident care through the nursing process of assessment, plans, implementation and evaluation. Directs nursing care in accordance with the policies and procedures of our organization. Ensures continuity of care by providing direct care and in supervising LVNs and CNAs.

Duties and Responsibilities
Orient and mentor new staff
Educate residents and their support group in a manner that respects their dignity and includes them in care decisions
Assess residents’ condition and provide necessary intervention upon admission, quarterly and when a change in condition occurs
Document all pertinent resident information and changes in communication logs, medical record, care plan, and MDS on a daily basis
Initiate and lead resident care conferences updating plan of care if needed
Participate in nursing meetings and Performance Improvement plan
Other duties requested by Director of Nursing
Perform technical and manual procedures including the administration of medication and clinical treatments according to physician orders and established standards set by Santa Teresita policy and quality practices within RN scope of practice
Serve as resource for LVNs and CNAs; assist LVNs and CNAs with medication and procedures per RN scope of practice
Respond to emergency situations according to policy and procedure, assuming a leadership role in these situations
May be designated to make rounds on all nursing units to assess for changes of condition, care planning, and document problems identified with appropriate follow-up for their shift
Review resident medical records in conjunction with Director of Nursing to assure that all psychotropic medications have appropriate informed consents and informed choice forms completed and the behaviors and medications are clearly documented in the care plan
Review monthly pharmacy consultant reports and assists the nursing staff to assure that all areas of concern are addressed and completed as indicated
May be asked to take a nursing station assignment if needed
May be designated to assist if needed with staff schedule in order to find replacement coverage including staff skill assessment and placement of staff on daily assignment sheet
Promote staff performance through education, acknowledgement, and recognizing effective performance; document counseling when needed for adverse performance

Skills and Job Requirements
Treat and converse with residents, visitors and staff in a manner that exhibits respect, professionalism and tact
Self-motivated in time management and prioritization of multiple residents needs and/or tasks in order to organize workload so that all functions are completed appropriately
In-depth understanding of the aging process, its physical, psycho-social and spiritual consequences and the ability to apply that understanding in practical support of each individual resident
Critical thinking skills, decisive judgment
The ability to work with minimal or no direct supervision in a fast-paced environment
Able to remain calm in an emergency and / or stressful situation and gives confidence that the situation is under control to others
Ability to provide IV therapy, treatments and procedures according to RN scope
Attends all in-services and other training sessions required by this position
Physical and Sensory Requirements
Must be able to see and hear or use prosthetics that will enable these senses to function adequately to assure that the requirements of this position can be fully met
Ability to lift a minimum of 25 pounds alone, and a minimum of 50 pounds in partnered lifting to a height of 5 feet
Ability to push/pull or move a minimum of 250 pounds for a distance of 1, 000 feet
Ability to reach overhead, bend, squat and crouch
Ability to stand and walk about for long periods of time
