Case Managers are responsible for assisting individuals to make full use of Behavioral Health Services and natural community supports which will help individuals achieve a more independent and fulfilling life. Services provided will include developing a partnership with the individual to assess needs, obtain services and resolve crises. Through this partnership the individual and case manager work together to plan, coordinate, monitor, and advocate for the services and support needed to achieve the individual's goals.   Position Requirements BSW with a preference for 1-2 years experience in the Behavioral Health field.
