Description

Maryland LMSW or LCSW-C needed for full-time, temporary position as Director of Social Worker in a long term care/skilled nursing facility setting in Baltimore.

Job responsibilities include:

  • Manages the total program of social services and spiritual services, ensuring that the delivery of social services meets quality standards. Ensures that staff completes social service documentation (progress notes, care plan, Minimum Data Set (MDS), etc.) by conducting chart audits and interviews with residents. Prepares Social Services department for quarterly Pre-Admission Screening and Resident Review (PASRR).
  • Serves as a member of the Senior Leadership team, representing the interest of the residents. Advocates for appropriate care and treatment of residents through the development and implementation of policies, and the education of residents, staff and family members regarding residents’ rights, as well as consultation with the Long-term Care Ombudsmen Program.
  • Ensures that Keswick policies and procedures meet social servicesregulatory requirements: discussion of code status within 72 hours of admission; complete social services assessment within 7 days of admission; social services sections of the comprehensive assessment tool (MDS) are completed as required; resident problems regarding mood, behavior, and pyscho-social/emotional needs are identified in the resident’s care plan; progress notes are written as needed but at least every 90 days in long-term care and according to schedule in rehabilitation care; discharge planning is addressed upon admission and at least quarterly thereafter. Ensures that residents are assisted in understanding their rights regarding Advance Directives and that they are offered information, resources and paperwork to make their wishes known.
  • Establishes and administers quality assurance procedures and modification of these procedures where appropriate.
  • Works with other medical professionals to identify all social, economic and emotional needs of residents and works closely with residents, staff, family members and other outside agencies to implement plans that address these needs. Provides in-service education to social services staff and other clinical staff on social services issues and Maryland regulatory requirements regarding Advance Directives, Surrogacy and MOLST.
  • Hires, trains and evaluates subordinate staff. Supervises the work of social services and spiritual services staff to ensure competency and performance. Provides guidance and direction.
