The incumbent provides and coordinates remote nursing care for cardiology patients recently discharged from the hospital as well as patient with chronic conditions. The candidate collaborates with the health care team to develop goals, review results and execute nursing interventions through coordination of care needed to maintain optimal health of patients with chronic disease. Essential Job Functions: Providing telephone services and or in person care as an extension of the Physician/APP staff. The role includes navigating multiple EMR and providing a plan of care, which encompasses a range of care coordination services, including health interviews with patients, family members and care givers. In addition, incumbent works with community providers and services to facilitate access to care.   Responsibilities: Meet goals for production to register and maintain patient enrollment in remote services in accordance with the practice guidelines. Creates and maintain comprehensive patient care plan, while encouraging health and wellness. Reviews and update patient care plans according to chronic condition protocol, medication reconciliation and communication with practice patients enrolled in remote service programs. Continually educates and re-educates patients regarding medical condition, the importance of compliance with medications, their physician’s plan of care and the need for continued management of disease. Candidate must thoroughly document the care plan with updated information, i.e. test results, education, social influences/barriers, etc. Maintains strict patient confidentially and follows SBM HIPAA compliance and guidelines. Documents all communication and assures documentation supports billing for appropriate level of remote services. Develop policy and procedures as necessary to improve efficiency, quality and consistency of remote service procedures and nursing care. Continues education to improve and maintain professional development. Stays with current regulations and keep staff up to date. Assist with disability forms request Educate all staff on OSHA regulations and guidelines and maintains practice compliance with OSHA regulations and compliance. Performs other duties as assigned   Category Nursing   Exempt/Non-Exempt Non-Exempt   Location Cardiology- NSC East Setauket   Full-Time/Part-Time Full-Time   Position Requirements Qualifications: Graduate of an accredited school of nursing. Possession of a NY State Registered Nurse license. Maintain current ACLS and BLS certification. Minimum two years’ experience in-home care or clinical setting. Experienced with medical practice EMR Cardiology Medical Practice experience preferred. Remote clinic service experience highly desired. OSHA regulations and compliance knowledge.   In accordance with the New York State Department of Health (DOH) order that all hospitals and nursing homes “continuously require all covered personnel to be fully vaccinated against COVID-19.”  Upon receipt of a conditional job offer, candidates who are not already vaccinated must obtain the first dose of the vaccine within three (3) calendar days of job offer and must obtain any subsequent doses in accordance with the vaccine protocol. The order allows for limited medical exemptions with reasonable accommodations, consistent with applicable law.   Physical Demands: May spend a great deal of their professional day walking, standing, moving and manipulating equipment, as well as lifting and moving objects, equipment and supplies. Ability to lift 20 pounds. Moving, lifting or transferring of patients or objects may involve lifting of up to 50 pounds, as well as assist with weights of more than one-hundred (100) pounds.
