Clinician - OPTICS 


Seeking to hire a Full-Time Clinician for the OPTICS Department

Job summary

  • Responsible and accountable for providing direct behavioral healthcare services for assigned client caseload
  • Provides individual, group, couples, and family therapy in order to increase stabilization
  • Documents service outcomes as per CarePlus and payer’s policies and procedures
  • Assesses and evaluates clients at intake and/or admission, during ongoing treatment and/or service delivery, and at discharge and/or transfer  
  • Formulates service plans, with relevant goals, behaviorally measurable objectives that serve as indexes for evaluating treatment progress, and realistic time frames for achievement 
  • Provides services and supportive interventions, including wellness activities, case management, linkage to community resources, and provides inter-agency coordination of treatment 
  • Responds to clients for crisis intervention  
  • Documents services (i.e. assessments, treatment plans, progress reports, and discharge summaries) in clinical record on a timely basis with all entries authenticated and time/dated 
  • Plans for, communicates, and documents discharge/aftercare/transfer plans for clients completing treatment and/or accessing other services in the continuum   
  • Functions as a member of an interdisciplinary team in the assessment and/or evaluation of clients, articulates in written documentation the integration of client history and assessment information and actively participates in program and clinical team meetings, provides support for team colleagues, and engages in program-related performance improvement processes and activities 
  • Assists client with concrete services and advocacy, interfaces with other systems on behalf of client, provides, engages in inter-organizational care coordination, and follow up on referrals when indicated by initial and on-going assessment  
  • Assists with problem solving for individual and family member treatment and supportive interventions  
  • Provides wellness and educational/supportive group activities with clients and/or family members focused on mental illness, chemical dependency/addictions, and/or dual diagnosis 
Position Requirements


  • Master's degree in a behavioral health discipline from an accredited college or university and documented knowledge of at least two (2) years’ work experience with substance use populations.  
  • Preferred Qualifications: LCSW, LCADC, experience working on multi-disciplinary teams 
  • Must possess a valid driver’s license and must be insurable by Agency’s insurance standards.
