What You Would Be Doing

Provides the full scope of primary care services that fall under his/her field of training, including but not limited to diagnosis, treatment, coordination of care, preventive care, and health maintenance to patients. In addition, a Primary Care Physician is a member of the Boundless Health (BH) team. In that capacity, the Physician is expected to be familiar with the health center's philosophy, goals, and objectives and work collaboratively to achieve them. They must be mindful of, and comply with, all health center policies and procedures, as well as applicable state and federal regulations.

  • Provides comprehensive medical services to patients presenting for care at BH continuingly; examines patients using standard primary care medical equipment.
  • Where indicated, obtain a complete medical history and physical data on patients.
  • Establishes diagnosis and manages accordingly.
  • Reviews patients record and incorporate appropriate information into BH electronic health record.
  • Manage acute and minor urgent health conditions, including minor surgical procedures.
  • Orders or executes diagnostic testing to obtain definitive diagnoses.
  • Reviews incoming reports (e.g., lab, x-ray, and EKG) signs, dates, and follows-up in a timely manner.
  • Educates patients and/or caregivers about necessary and beneficial immunizations based upon patients’ age and health conditions.
  • Provide or directly oversee services and procedures appropriate in the outpatient setting.
  • Educate patients regarding health and illness prevention.
  • Recommend community resources to meet patient and family needs.
  • Administers or prescribes treatments and medications and instructs patients on proper utilization.
  • Refers patients to a medical specialist for consultant services when necessary for patient’s well-being.
  • Adhere to established clinical protocols and procedures.
  • Assist in the updating of BH protocols and procedures as requested.
  • Maintain adequate and legible patient medical records in the electronic health record.
  • Supervise or participate in patient emergencies as required.
  • Observe all infection control procedures.
  • Fulfill HRSA and health center productivity standards, including Uniform Data System (UDS) measures.
  • Follow established health center procedures for patient billing and reimbursement.
  • Participate in staff medical education programs and patient health education activities as required.
  • Report any discrepancies, irregularities, and/or violations involving clinical care, administrative procedure, billing issues, or other BH protocols to the appropriate personnel.
  • Maintain professional competence by completing appropriate specialty requirements of CME.
  • Maintain current certifications.
  • Participate in peer review, quality assurance, provider meetings, and other clinical meetings.
  • Performs telephone on-call duties as assigned.
  • Participates in weekend clinic hours as assigned.
  • Participates in covering legal holiday on-call telephone coverage.
  • Assist with the training of assigned staff.
  • Maintain the strictest confidentiality in accordance with HIPPA and BH regulations.
  • Participate in professional development activities and maintains professional affiliations.
  • Successful completion of BH credentialing and privileging review upon hire and re-credentialing.
  • Work with Behavioral Health Counseling staff to provide collaborative care for patients.
  • Actively manage the assigned panel of patients.
  • Work collaboratively within and between the primary care teams to support the spirit and the mission of Boundless Health.

Work Environment
This job generally operates in an office and clinical environment but may also require remote work as necessary.

  • Frequent exposure to communicable diseases, toxic substances, medicinal preparations, or other conditions common to a health center environment.
  • Participates in on-call rotation as appropriate (one night per week, one weekend per month, legal holiday coverage; exact schedule is determined upon hire).
 
Position Requirements

Required Education/Experience:

  • Graduate of approved and accredited medical school.
  • Licensed to practice medicine in the State of Ohio or licensed in another state with the ability to obtain licensure in Ohio.
  • Board Certified in Family Medicine, Pediatrics or Internal Medicine.
  • Eligible to obtain and maintain a federal and state narcotic registration (DEA License).
  • At least two years proven successful experience as a practicing clinician.
  • Strong community health/public health orientation, experienced inpatient care management and demonstrates managed health care knowledge and experience.
  • Working knowledge of ICD-10 or most current coding system and correctly assigning Evaluation and Management (E/M) codes for primary care.
  • Experience with electronic health record systems of care.
  • Experience in a multiple provider medical practice.

Preferred Requirements:

  • Familiarity with federally qualified health centers or FQHC Look-a-Likes. Experience working in a not-for-profit organization with annual operating margin goals.
  • Experience working with low-income, multi-ethnic populations.
  • Experience in dealing with patients with behavioral health problems or intellectual and developmental disabilities complicating their medical care.
  • Skill in Microsoft Office Applications: Word, basic Excel, and Outlook.

Physical Demands

The physical requirements described here are representative of those that an employee must meet to perform the essential functions of the job successfully. While performing the duties of this job, the employee is regularly required to sit; use hands to manipulate objects, tools, or controls; reach with hands and arms, and talk and hear. The employee must frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 25 pounds. This job's specific vision abilities include close vision, distance vision, peripheral vision, depth perception, and the ability to adjust focus.

Travel

Travel would occasionally be needed to fulfill organizational needs.
