Responsibilities include and are not limited to:

  • Supervision of daily tasks of the Intake Coordinator and Clinic Administrative Assistant
  • Act as a liaison between the agency and other providers to increase referrals
  • Work collaboratively with all departments to ensure referrals are provided promptly to appropriate services
  • Partner with Medicaid Transformation to provide services to staff and consumers
  • Utilize knowledge of clinical needs and program capabilities while screening referrals
  • Provide services in a variety of environments including the office and home of person servicing
Position Requirements

Minimum Qualifications:

Persons in this position must:

  • Hold Qualified Professional status: Must have a bachelor’s degree from an approved accredited institution; human services preferred. Spanish speaking an asset.
  • Experience providing case management.
  • Knowledge of DSM-V diagnoses, provider service options and community resources required.
  • Demonstrates high moral and ethical character and be free of any substantiated charges of abuse or neglect on the NC Healthcare Registry.
  • Possess valid, unrestricted, state-appropriate driver’s license and unrestricted, valid insurance for person vehicles utilized for work.
  • Must be open to work a flexible schedule including evenings and weekends as needed.
  • Must be able to communicate effectively.
  • Must be able to work in fast paced environment.

Equipment Used:

Standard office equipment: computer, fax, phone. Use of personal transportation.


Services are provided in a variety of environments, including the office; home and community of the person served. Work schedule can vary.
