Position is temp or temp to perm, 8:30-4:30 M-F. The Supports Planner position provides ongoing professional case management and advocacy work on behalf of individuals with complex medical needs and disabilities and their family members. Essential functions include: Advocates for clients, serving as the primary point of contact. Performs on-site visits (to homes, nursing/medical/assisted living facilities) and telephonic care coordination, providing supports planning and troubleshooting. Facilitates transitions from nursing facility or assisted living facility to community setting. Assists individuals with developing life goals and monitors progress to ensure health and safety in the community.   Position Requirements Work experience pertaining to complex case management for individuals with chronic conditions and disabilities and knowledge of Medicaid/Medicare, housing, and home and community based services are desirable.   This position is currently not accepting applications.
