Email Opening Title Health Promotion Specialist  Description Job Summary The Health Promotion Specialist improves the health of the Confederated Tribes of the Umatilla Indian Reservation community through planning, implementing, and evaluating health status, promotion and disease prevention programs, with an emphasis on the culture of the community. All duties and responsibilities will be performed in accordance with the Yellowhawk Mission and Vision statements. Essential Job Functions The focus of the Health Promotion Specialist is to develop the health awareness, in a culturally responsive manner, of individuals and families living with or at risk for chronic disease and empower them to make healthy choices; to increase opportunities for physical activity, nutrition information and health education. Is an active team member of the Pamanaknúwit Team (health promotion) and works towards meeting quarterly objectives in the Yellowhawk Annual Work Plan. Collaborate with the Pamanaknúwit Team to identify and implement strategies to be used to assist community members adapt a healthy lifestyle (increased physical activities, access to traditional and healthy foods, breastfeeding, and nutrition) Coordinate educational/health promotion events and opportunities, such as the Yellowhawk Annual Fun Run. Collect and use data to identify and implement health promotion strategies. Write and produce leaflets, posters, videos and brochures to aid health promotion in the community. Identifies areas of improvement through performance management tracking and conducts quality improvement studies. Requires some grant management, data collecting and reporting. Assist with Diabetes Prevention Program. Coordinate physical activity schedule, and may lead group exercise classes. To assist with healthy cooking classes for individuals on a monthly basis. Transports patients/clients as assigned. Provides information and education to the tribal community on topics that may include: basic nutrition, physical activity recommendations, weight loss, chronic disease prevention and management through one-on-one counseling and group activities. Other duties as assigned.   Position Requirements Minimum Qualifications An Associate’s degree in public health, health education, nursing, and/or health fitness with 2 years of relevant and applicable work experience OR High School Diploma or equivalent with 4 years of relevant and applicable work experience. Demonstrated experience and knowledge of health promotion and working with people who have chronic health conditions. Demonstrated ability to maintain strict confidentiality. Demonstrated excellent customer service skills. Demonstrated ability to communicate effectively, both in writing and oral, in any situation, with peers, staff, management, patients and the general public. Must obtain and maintain a CPR training certification within 90 days of hire. Must possess a valid driver’s license and maintain insurance requirements to operate General Services Administration vehicles. Must submit to and pass a reference and criminal background check. Must submit to and pass a pre-employment drug and alcohol screening.
