Description

POSITION SUMMARY

Responsible for implementing the school-based prevention programs outlined in the Special Diabetes Program for Indians (SDPI) grant. Plan, organize, and administer health-related fitness assessments in local schools to trend childhood obesity rates of school age children/adolescents. Establish and maintain working relationships with local schools to provide evidence-based school health practices to improve the health of students, staff, and parents. Provide technical assistance to schools implementing school health programs and services. Provide resources, tools, and trainings for schools in the areas of diabetes prevention best practices. Collaborate and communicate with clinical providers, hospital departments, and other public health organizations. Knowledge and understanding of electronic health records. Other duties to meet department/organization grant goals and objectives. Knowledge of local tribal philosophy and cultural practices is a must. Incumbent must be willing to work evenings and weekends, and as needed. This position is grant funded.

ESSENTIAL FUNCTIONS:

  1. Responsible for the coordination, implementation, and evaluation of school diabetes prevention activities (Fitnessgram assessments, Classroom Movement Breaks, PAK Training, etc.) as outlined in SDPI goals and objectives.
  2. Provide leadership and guidance to local schools in maintaining or establishing their respective school health programs.
  3. Serve as a resource on the use of evidence-based physical education curricula (S.P.A.R.K., Physical Best, etc.).
  4. Provide school educators training opportunities to build capacity in the delivery health promotion and youth physical activity.
  5. Host monthly School Health Team Leader meetings and administrator meetings to maintain or gain school support for school health services and programs.
  6. Coordinate school physical activity programs (Classroom Movement Breaks) with local schools to increase student physical activity levels.
  7. Collaborate and meet regularly with local public health agencies, tribal chapters, government agencies, community groups, health care providers, county agencies and other organizations.
  8. Assist with contractors and contracts that support school FitnessGram assessments and other school health-related activities.
  9. Responsible for outcome reports and analysis of the school-based prevention programs. Maintains accurate participant records and prepare a variety of monthly and quarterly reports on program activities and operations and gather statistical data as needed or requested. Identify opportunities for improvement and review with the HPDP Assistant Program Director.
  10. Assists with reports and special projects for the HPDP program.
  11. Arranges for necessary supplies, transportation, space, services, equipment, and promotion of school-based prevention programs.
  12. Incorporate and integrate culturally appropriate concepts, modifications and activities when delivering health promotion/diabetes prevention education and services.
  13. Assists HPDP Fitness Center operations such as registration; orientation/training; equipment maintenance; and, exercise room supervision.
  14. Supports culture of safety and take extra steps to ensure safety of all. Report injuries, safety concerns, and any emergency event as stated in TCRHCC policy.
  15. Demonstrate knowledge of emergency first aid procedures and administer emergency exercise related medical treatment as needed. Participates in mandatory in-services regarding Fire Safety, and OSHA. Administers emergency exercise related medical treatment as needed and knowledge of emergency first aid procedures.
  16. Deliver effective communication toward supervisor, program staff, program partnerships, hospital staff, and community members. Communication abilities include written and verbal skills that demonstrate the ability to motivate or encourage others to alter lifestyles and group dynamics.
  17. Aids other departments within Community Health Division.
  18. Understands and adheres to established compliance, safety, and quality assurance standards.
  19. Ensure Proper PPE is always used while on duty. Face surgical mask is to be worn in all hospital areas. Proper handwashing is required. If water is not available use hand sanitizer. Social distancing will be practiced by department.
  20. Proper PPE is used inside assigned Clinics or Unit. NIOSH-approved N95 mask filtering face piece respirator or higher, is available, this included eye or face masks shield, gloves, and isolation gown.
  21. Complete all donning and doffing tasks in a safe acceptable method and discard of used PPE accordingly. Complete provided training.
  22. Performs other duties as assigned.

PHYSICAL:

Must be able to exercise physical exertion when overseeing activities, including all types of repetitive bending, standing, floor exercises, and lifting. Must be able to continually sit, stand, walk, bend, maintain balance and reach for a prolonged period as well as frequently drive, crouch, twist and occasionally climb and kneel. Must be able to continuously lift to 24lbs, frequently lift to 75 lbs. and occasionally lift over 100 lbs. of equipment. Must be able to push and pull continuously up to 24 lbs. and occasionally over 100 lbs. of equipment. Travels and transports equipment/supplies to various communities and conducts program activities in rural communities within the Tuba City Service Area. Sensory requirements for position include far vision, near vision, color vision, depth perception, seeing fine details, hearing normal speech, hearing overhead pages, and using the telephone.

MENTAL:

Must be able to prioritize, use good judgment, and be able to coordinate a variety of projects while being frequently interrupted. Must have a non-judgmental attitude and be comfortable working with low-resource individuals/communities. Must demonstrate cultural sensitivity. Must be able to continuously cope with high levels of stress, make decision under high pressure, concentrate, handle high degree of flexibility, handle multiple priorities in stressful situation, work alone, demonstrate high degree of patience, adapt to shift work and work in areas that are close and crowded, frequently manage altercations and occasionally cope with anger, fear, hostility of others in a calm way. Must accept a flexible schedule to meet unit needs.

 
Position Requirements

NECESSARY QUALIFICATIONS

Education:

  • Bachelor’s degree in a Physical Education, Exercise Science, Public Health or related degree

Experience:

  • Must have two (2) years’ experience in Physical Education, Health Promotion or community activity programs that applies knowledge of principles, methods, and techniques to plan, implement and evaluate health programs.

Certification:

  • Must have and maintain Basic Life Support Certification (BLS) from the American Heart Association
  • Training or Certification in Group Fitness or Personal Fitness
  • Must maintain and have an unrestricted current valid state driver’s license

Other Skills and Abilities:

A record of satisfactory performance in all prior and current employment as evidenced by positive employment references from previous and current employers. All employment references must address and indicate success in each one of the following areas:

  • Positive working relationships with others
  • Possession of high ethical standards and no history of complaints
  • Reliable and dependable; reports to work as scheduled without excessive absences
  • Basic knowledge data collection, analysis, practice, and ethical considerations
  • Proficient with the following software programs: Excel, Word, Power Point, Outlook, Adobe, and/or Access
  • Skilled and comfortable in conducting presentation/public speaking
