Description

Open Door Community Health Centers is looking for a Latino Health Coordinator (Promotore) to facilitate access to health care for the Latino community.  We are seeking an experienced individual with a diverse medical and community background to help organize and engage Latino families in their health and health care. The Latino Health Coordinator works as an interpreter when needed and is utilized as an ally to educate Latino families regarding their healthcare decisions. The position requires a well-organized individual who is willing to reach out to patients and families to ensure all accommodations are met as fully as possible. The Coordinator works as part of the health care team for patients alongside providers and clinical support staff to ensure comprehensive quality care. This position requires strong communication skills in both English and Spanish, and the ability to learn, comprehend and explain medical terminology. Strong ties to the Latino community, excellent Spanish and English, and good computer skills necessary; prior experience in a health care or community organization preferred.

 
Position Requirements

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Assist patients in accessing specialty care by making appointments, reminder calls and arranging transportation when indicated.

  • Contact patients for follow up when needed.

  • Provide medical translation at evening clinics and specialty appointments.

  • Provide patient and community education about diabetes, breast and cervical cancer, caries prevention, home glucose monitoring, importance of physical activity and healthy diet.

  • Facilitate support groups.

  • Enroll children in health programs such as CalKids, Healthy Families and Medi-Cal.

  • Act as a bridge to community resources, including patient assistance programs for prescriptions and St. Joseph Hospital Charity Care, Breast Health Project, Paso a Paso, Food Bank, Mental Health, Rape Crisis Team and Domestic Violence Services, School IEP, Special Education Programs, etc.

  • Publicize Flouride Varnish Clinics, Diabetes Support Groups and other Latino Health Promotion events via press releases, flyers and word of mouth.

  • Attend staff and team meetings,

  • Adhere to ODCHC attendance policy

  • Other duties and responsibilities as designated by provider/supervisor.

QUALIFICATIONS:

  • Ability to work as a member of a team in order to communicate and/or solicit pertinent information to and from other team members and support team decisions.

  • Ability to communicate effectively and exercise sound and responsible judgment.

  • Excellent interpersonal skills, written and verbal.Ability to establish constructive working relationships with all levels of management and employees in a staff of varied and diverse backgrounds.

  • Ability to handle difficult or confrontational situations in a calm, consistent, and equitable manner.

  • Ability to write routine reports and correspondence.

  • Ability to accurately chart patient information.

  • Ability to promote effective communication between the patient and the primary care provider.

EDUCATION and/or EXPERIENCE:

  • High school diploma or GED.

  • Experience working with Latino population.

  • Experience in a medical environment.

CERTIFICATES, LICENSES, REGISTRATIONS:

  • Current CPR certification

PHYSICAL DEMANDS:

  • Speaking and hearing sufficient to communicate effectively by phone or in person, at normal volumes.

  • Vision adequate to read correspondence, computer screen, forms etc.

  • Good manual dexterity.

  • Ability to sit or stand for extended periods of time.

 
Wage Range Wage dependent on experience; $17.05/hour minimum.
