FT (M-F, 9-5), permanent position available for an LMSW/LCSW to provide hospice services to terminally ill patients and their families. The social worker visits patients and families in their homes to provide assessment, resources, education and support. Also responsible for coordination and implementation of hospice plan of care and participation in interdisciplinary team meetings.   Position Requirements Some experience with medical/hospital, LTC/nursing home, hospice or other similar setting is required.
