We provide support services and advocacy to 400 adults and children and their families in Harford and Cecil Counties. The Arc supports people with intellectual and developmental disabilities to create lives that mirror yours and mine: students attending neighborhood schools, adults going to work and living in their own homes, and individuals enjoying the social and recreational opportunities in their communities. GENERAL DESCRIPTION

 

Under the supervision of the Director of Vocational Services, the Transition Coordinator’s primary responsibilities include representing The
Arc for transition activities and facilitating successful transition for students to Arc services.

 

QUALIFICATIONS

 

  1. Bachelor’s Degree in Human Service or related field preferred.
  2. Three (3) years of technical experience working with people with developmental disabilities.
  3. Excellent written, oral and organization skills.
  4. Access to reliable transportation with a valid Maryland driver’s license and an acceptable driving record (no more than 2 points).
  5. Basic computer skills required

 

 

JOB SPECIFIC FUNCTIONS AND RESPONSIBILITIES

 

  1. Serve as an effective representative of The Arc Northern Chesapeake Region’s for transitioning student activities and to individuals and organizations involved in transition.
  2. Develop and deliver successful presentations regarding Arc services to transitioning students (and their representatives).
  3. Develop funding and transitioning plans required for initiating services to transitioning students.
  4. Coordinate activities involved with transition from school to Arc services for transitioning students.
  5. Provide case management for individuals as assigned.
  6. Complete assessments as required.
  7. Complete other duties as assigned.
