Seeking to hire a Full – Time Clinician for the Dual Diagnosis Department

Job summary

  • Provides direct behavioral healthcare services for children, adolescents, and adults
  • Documents service outcomes as per Care Plus and payer’s policies and procedures
  • Assesses and evaluates clients at intake and/or admission, during ongoing treatment and/or service delivery, and at discharge and/or transfer
  • Provides services and supportive interventions, including case management, linkage to community resources, and provides inter-agency coordination of treatment
  • Responds to clients for crisis intervention
  • Documents services (i.e. assessments, treatment plans, progress reports, and discharge summaries) in clinical record on a timely basis with all entries authenticated and time/dated
  • Plans for, communicates, and documents discharge/aftercare/transfer plans for clients completing treatment and/or accessing other services in the continuum
  • Assists client with concrete services and advocacy, interfaces with other systems on behalf of client, provides, engages in inter-organizational care coordination, and follow up on referrals when indicated by initial and on-going assessment
  • Assists with problem solving for individual and family member treatment and supportive interventions Supervises students or junior staff as assigned by supervisor, documents supervision as required
  • Provides educational/supportive group presentations and discussions with clients and/or family members focused on mental illness, chemical dependency/addictions, and/or dual diagnosis
