Description

Director of Social Work needed for full-time, temporary (6 month) position in Philadeplhia County.

The Director of Social Work is responsible for the direction and leadership of operational, programmatic, and personnel activities for Sub-Acute and Therapy Social Work services as they relate to Long Term Care (LTC) facilities (SNF, PCH, etc.). The Director works closely with a variety of stakeholders, coordinating the activities of subacute social services across the enterprise to ensure all parties invested in the care of Patients/Residents meet mutual expectations of accountability for quality and efficiency of care.

 

 
Position Requirements

PA LSW or LCSW with 5+ years clinical o macro experience. 2+ years leadership experience, post-acute care (SNF, IRF, Home Health). Knowledge and understanding of post-acute services; experience in skilled nursing facilities, personal care homes, and other post-acute care, community, and/or behavioral health settings. Effective analytical ability: able to develop and analyze options, recommend solutions to and solve complex problems and issues. Understands public and private health insurance for long-term care; able to assess and understand the influence of reimbursement on LTC facility resources and operations. Understands regulatory requirements for licensure of LTC facilities; understands the similarities and differences between the licensure requirements of SNF, PCH, ALF, Boarding Homes, etc.

Work is accomplished without considerable direction. Exercises judgment in selecting methods, techniques, and evaluation criteria in obtaining results. Exerts significant latitude in determining objectives of assignment. Takes calculated risks with consultation from the expert. Office Environment and Ability to effectively and safely work from home in a secure environment, free from distraction.
