MEDICAL RECORDS COORDINATOR JOB DESCRIPTION

Supervision Received: Works directly under supervision of the Program Director; receives general guidelines and instruction from the Medical Services Coordinator (MSC) and Program Managers (PM). The Program Director or CEO/President are available to resolve any work issues that may arise.

General Description of Duties: The Medical Records Coordinator is responsible for monitoring and notification of medical services and maintenance of individual medical database and information for individuals residing in various settings including host homes, family caregiver homes, and unit/PCA homes. You will also complete assigned day to day office duties including back up to receptionist and computer work as assigned. Assisting and working closely with the MSC and PM(s) on projects and assigned duties.         

EXAMPLES OF JOB DUTIES:

INDIVIDUAL RECORD MAINTENANCE:

  1. Maintenance of individual medical appointment record and database.  Ensure that database is always current.
  2. Weekly review of medical consultations from staff, HHIC, and family caregivers ensuring completion and follow-up has occurred.  Input information into database and verify that all appointments have needed attachments per CCA policies.
  3. Send Medical Services Coordinator monthly appointments that need to be scheduled for unit individuals based on the medical database.
  4. Follow-up with unit PMs on-going or as needed basis for resolution of outstanding paperwork due from staff/HHIC.
  5. Send out monthly medical corrections to staff, HHIC, family caregivers, and assigned PMs. Review for completion after the deadline to ensure all follow-up has been completed. If not, follow-up with PM on missing information for their caseloads.
  6. Support with processing invoices for vision and medical billing for all services, per timelines, as needed. Work with PMs to resolve outstanding approval requests for CCBs.
  7. Be familiar with Medical Services Coordinator’s duties and provide backup as needed. Support/provide coverage for the medical services coordinator with medication counts/orders as needed.
  8. Complete quality assurance monitoring on medical files for family caregivers, HHIC, and units as requested.
  9. Maintain medical adaptive equipment database for units, HHIC, and family caregivers and ensure that annual/as needed inspections are completed.
  10. Work with Intake Coordinator and assigned PM for new intakes to ensure that all medical documentation is received. Request any missing documentation and ensure all requested information is received and stored in correct location.

GENERAL OFFICE DUTIES:

  1. Assist office staff in completing projects as assigned.
  2. As requested, research and order new medical supplies based on CCA/individual’s needs.

RECEPTIONIST DUTIES:

  1. Answer all incoming phone calls, routing or taking messages as indicated, ensuring messages are complete and clear, and relayed in a reasonable time frame.
  2. Greet visitors in a professional and courteous manner, ensuring that they are assisted as needed, and that the front area is not left unattended.
  3. Ensure pickup and delivery of incoming and outgoing mail daily and ensure mail is distributed as per agency guidelines.
  4. Ensure coffee stations, bathrooms, conference, and general office areas are neat and clean on an ongoing basis.
  5. Ensure all needed supplies are kept in the bathrooms and kitchens on an ongoing basis.
  6. Ensure employees sign out unit supplies, fill out supply check list and, ensure the information correct by initialing the checklist.
  7. Maintain inventory of unit and office supplies, ordering and receiving such supplies as assigned.
  8. Grocery cards, and PI&E checkout as needed.
  9. Assist office staff in completing projects as assigned.

EXPECTATIONS FOR COMPLETION OF JOB DUTIES:

  1. Display professional appearance and behavior, interacting with other staff, individuals, parents, and service providers in a cooperative and friendly manner as observed by supervisor.
  2. Punctual and dependable at reporting to work.
  3. Perform job duties with a positive attitude, responding to peak work period without loss of quality, accuracy, and thoroughness.
  4. Uphold individual rights by following individual rights and confidentiality guidelines and agency abuse, neglect, mistreatment, and exploitation policy.
  5. Provide positive role model to staff and individuals always, avoiding use of profanity or derogatory slang, arriving to work in proper attire, modeling good problem solving and social interactions.
  6. Attend regular and as scheduled meetings and training as assigned.

 

 
Position Requirements

GENERAL REQUIREMENTS:  High School Diploma or GED. Previous experience in related field and/or position preferred. Proficient with Microsoft Office software.  Excellent organizational skills and time-management skills with strong attention to detail.  Solid business communication skills both verbal and written.  Be able to prioritize job duties and meet deadlines.  Work independently as well as the ability to participate and contribute as a team member. Must be professionally positive, courteous, and friendly with all interactions.  Valid Colorado driver’s license and insurance and ability to pass a background check.
