Case Manager - ICMS

Seeking to hire a Full Time Case Manager for ICMS

Job summary

  • Engages consumers and their families
  • Facilitates and coordinates psycho-educational services (including groups) for consumers and their family members on a variety of topics relating to wellness (physical and mental), health promotion, prevention, skill-building and related health and wellness/recovery topics
  • Offers on-site as well as outreach services (including transport of consumers)
  • Fulfills case management services, including, but not limited to coordinating or directly facilitating linkages to services, service coordination, treatment enrollment assistance, etc.
  • Ensures consumers have community supports in place (and engage in service planning as appropriate) that fulfill a positive corroborative role
  • Communicates observations and information about the consumer in the consumer's service record and to team members
  • Engages in supportive counseling and crisis intervention
  • Participates in assessment and evaluation of consumers at intake/admissions, during ongoing recovery/service delivery and at discharge
  • Collaborates with team and collaborative treatment providers to establish comprehensive bio-psycho-social assessments and care plans
  • Provides input into formulation of consumer-centered care plans in collaboration with team members, containing relevant goals, measurable objectives (that serve as indicators for evaluating progress) and realistic time frames for achievement; performs reviews of progress and evaluates effectiveness of goals/objectives
  • Works with administrative staff to ensure that all services are documented based on billing and regulatory standards; collects information on consumers and reflects all demographic data
  • Participates in quality assurance activities and cross-functional action teams

Qualifications

  • Bachelor's Degree in Social Worker OR
  • Bachelor's Degree in Mental Health or related field, plus one (1) year post BA experience in Mental Health or related field
  • Extensive knowledge of community services
  • Must be able to work a flexible work schedule
  • Valid driver's license insurable by agency's automobile insurance
