Share Email Opening Title CCBHC Health Navigator  Description Seeking to hire a Full Time CCBHC Health Navigator for Outpatient Clinical Services Job summary Serves as a liaison role for assigned partner organization(s) Completes data collection requirements for projects and reports back to leadership Works with administrative staff to ensure that all services are documented based on billing and regulatory standards; collects information on consumers and reflects all demographic data Participates in quality assurance activities and cross-functional action teams Fulfills case management services, including, but not limited to coordinating or directly facilitating linkages to services, service coordination, treatment enrollment assistance, and so forth Documents services/client information in clinical record on a timely basis with all entries authenticated and time/dated Ensure all project outcomes are met and reports are submitted in a timely manner Ensures consumers have community supports in place (and engage in service planning as appropriate) that fulfill a positive corroborative role Engages in supportive counseling and crisis intervention Participates in assessment and evaluation of consumers at intake/admissions, during ongoing recovery/service delivery and at discharge   Qualifications Bachelor’s Degree from an accredited college or university with a major in Social Work, Psychology, Counseling, Public Health, Healthcare Administration or related field Experience working in public health, healthcare setting, or mental health agency is preferred Working knowledge of EHR functions (implementation, data collection/analysis, report writing) A valid driver's license is required, and must be insurable by agency's automobile insurance   CPNJ embraces a culture of diversity and inclusion in all of it policies and procedures.
