Summary of responsibilities include: Conduct psycho-social assessments on each dialysis patient and determine social work treatment plan compatible with a total medical treatment plan per CMS guidelines Track care plans for dialysis patients and coordinate care plan meetings with the interdisciplinary team Provide an education packet to new patients regarding their Rights and Responsibilities, Grievance Procedure, treatment options, and the importance of treatment compliance. Participate in administrative and staff programs as required, including attending staff meetings and in-service meetings.   Position Requirements Requires: Master’s degree in social work. Healthcare, case management, or dialysis-related background preferred. Must be licensed as an LCSW or LCSW-C Unencumbered driver’s license.
