With the belief that all people have the capacity to grow and change, we provide our services to people of all ages, cultures, races, religious preferences, genders, and sexual orientations in order to enhance wellness and recovery, thereby improving families, workplaces, and communities.   Position Care Manager   Description Our Clinic Care Managers are the critical link between the client, the healthcare system, and the community.  The Clinic Care Manager works to ensure that the client receives comprehensive and integrated care to support behavioral and health needs to attain goals that support independent living.  They offer proactive, integrated care coordination and care planning for improved care quality, access to services and resources, health and functioning of the patient, and quality of life.    Position Requirements Must have an AA/BA or Community Health Worker certification. Qualified applicant must be willing to transport patients in their personal vehicle.
