Social Work p.r.n. has an immediate Monday-Friday, full-time, direct hire opportunity for a LSCSW to serve as a Behavioral Health Clinician (BHC) for an integrated health clinic. The BHC will work with a multi-disciplinary team to meet the needs of primary care patients who may suffer form medical conditions and co-occurring mental health conditions. The BHC will provide mental health consultation, assessments, psychoeducation, crisis intervention and brief interventions.
Job responsibilities include, but are not limited, to the following:
Provide comprehensive assessment and diagnosis of the behavioral health condition and co-existing medical illness.
Assist primary care team with developing a treatment plan and brief interventions for co-occurring conditions.
Evaluate crisis situations and appropriate interventions.
Assist in the detection of “at risk” patients and develop plans of care to prevent further deterioration.
Provide education to patients, family, and staff on prevention and treatment techniques to improve self-care.
Commitment to expand access to care for underserved populations.
Other duties as designated.
