OVERVIEW: Promoting and protecting the health of the community. Coordinates and performs patient care in collaboration with interdisciplinary treatment teams for acute and chronic illness and issues; intervenes at key points to maintain optimum, independent, healthy lifestyles; assists/promotes healthy lifestyle changes and medical follow-up as necessary. Duties include home visits for health assessments and monitoring; community health education and public health.

POSITION STATUS: Full-time w/benefits

SALARY LEVEL: DOQ

WORK SCHEDULE: Full time 40/hrs. week

ESSENTIAL FUNCTIONS OF JOB:

  • Plan and deliver a community health program, taking into account the physical, social, spiritual, mental, and environmental factors that influence each individual, family and the community at large.
  • Provide nursing services and treatments to patients in the home, clinic, and/or the community setting. Determine patient needs and develop care plans that include a holistic and comprehensive approach.
  • Develop and monitor programs, activities and trainings that support health promotion, disease prevention, and community development.
  • Receive training in the Health Home program and deliver care within this service.
  • Coordinate and supervise Community Health Representatives.
  • Provide care management and skilled nursing visits in collaboration with the patients Primary Care Provider (PCP) to eligible individuals within the service area.
  • Identify high-risk patients and reach out to this population to promote health and wellness.
  • Participate in department wide planning, priority setting, and program evaluation.
  • Maintain professional standards; follow treatment policies of the Health Services Department, tribal, federal, state and local regulations.
  • Perform accurate and timely documentation of all patient interactions in the electronic health record.
  • Work with Lead Clinical Nurse, Triage Nurse and Medical Director to coordinate immunization events for children, adults, and document these efforts.
  • Provide assistance to other Tribal Services in direct care, coordination of referrals, and contract compliance issues.
  • Be aware of and collaborate with Tribal Social Services, Clinic Mental Health and Substance Abuse programs as well as other outside agencies. Make referrals as needed to these agencies for patients and/or patient family groups as needed.
  • Perform other related duties as assigned or needed.
 
Position Requirements

DESIRED QUALIFICATIONS:

  • Possession and maintenance of a current valid license as a Registered Nurse in Washington State
  • Bachelor’s Degree in Nursing (BSN) preferred
  • Five (5) years experience in care coordination, internal case management or Community Health Nursing preferred
  • Current CPR and First Aid certifications
  • Basic knowledge of and adherence to HIPAA and other related federal laws and regulations

CONDITIONS OF EMPLOYMENT:

  • Must have insurable driving record and a valid Washington State driver’s license within 30 days of hire
  • Must sign an oath of permanent confidentiality covering all client-related information
  • Must pass a criminal background check
  • Must have no history or evidence of alcohol or other drug misuse for a period of three (3) years prior to the date of employment within the department, and no misuse of alcohol or other drugs while employed within the department
  • Must maintain CPR and First Aid certification
  • The Health Services Director shall sign and date the job description to affirm the validity of the information herein. The employee shall also sign and date the job description to signify compliance with the conditions of employment, minimum qualifications, and an understanding of the job responsibilities.

PHYSICAL REQUIREMENTS:

  • Often physically demanding, including lifting occasionally up to 30 pounds
  • Must be able to work in a wide range of areas up to 8-10 hours per day, including but not limited to kneeling, stretching, squatting, sitting, standing, walking, and stooping

OTHER REQUIREMENTS:

  • Must be able to comply with the Lower Elwha Klallam Tribe Drug Free workplace.
  • Must be able to travel as needed.
  • Observe and apply strict confidentiality for patient care.
