The Medical Support Coordinator shall be currently licensed as a Psychiatric Nurse Practitioner (PNP) in NC and meet the requirements as specified in 21 NCAC 36.0800, approval and practice parameters for nurse practitioners, with at least three years full-time experience treating individuals with SPMI.

The Medical Support Coordinator / Psychiatric Nurse Practitioner will serve as a prescribing member of an Assertive Community Treatment Team that ensures service availability 24 hours a day, 7 days per week to persons determined to be so debilitated with their illness and/or unpredictable that they cannot be met effectively at a less restrictive level of care with available community services. In addition, this role will serve to ensure appropriate nursing and medical practice standards across all the agency ACT Team programs by providing oversight, training, consultation and follow-up on established standards of practice. The Medical Support Coordinator/ Psychiatric Nurse Practitioner will work closely with the interdisciplinary team in a leadership role collaboratively with the Team Lead and Psychiatrist, to include clinical supervision, management of mental health medications, clarification of mental health diagnosis on an on-going basis, assisting with linkage to primary care providers for general medical needs, assisting with crisis intervention, providing supports to currently involved social service agencies, and providing recommendations for linkage to needed social services agencies.

Psychiatric Care Provider Functions:

  1. Typically sees the individuals at least monthly for the assessment and treatment of the individual’s symptoms and response to medication including side effects. Frequency will vary for each individual, with the vast majority seen within 4 to 6 weeks of last appointment, with many of the contacts being in the community.
  2. Collaborates with the team leader in sharing overall clinical responsibility for monitoring individuals
  3. Provides treatment and clinical supervision to the team collaboratively with the Team Lead.
  4. Actively collaborates with nurses to develop and implement medication administration policies
  5. Procedures as well as oversee the medical care of beneficiaries that include regular screenings for medical conditions and assessment of wellness and health management.
  6. Educates non-medical team members on psychiatric & non-psychiatric medications, their side effects, & health-related conditions; provides diagnostic and medication education to beneficiary, with medication decisions based on shared-decision making.
  7. Regularly participates in daily team meetings and treatment planning meetings; attends daily team meetings in proportion to time allocated on team.
  8. Provides brief therapy (formal or informal).
  9. Provides psychiatric back-up to the program after-hours and weekends. (Note: may be on a rotating basis as long as other psychiatric care providers who share on-call have access to individual’s current status and medical records/current medications).
  10. Works with the ACT team leader to monitor each individual’s clinical status and response to treatment.
  11. Provides psychopharmacologic management of patients
  12. With the assistance of nursing and other members of the team, consults and collaborates with primary care providers to address general medical needs
  13. Meets the critical standards as outlined in the most current edition of the National Program Standards for ACT teams as established by the National Alliance for the Mentally Ill or US Department of Health and Human Services, Center for Mental Health Services.
  14. Provides assistance to the ACT team staff in the delivery of services in accordance to best practice standards as set forth by the appropriate agents listed above
  15. Assists in training new employees
  16. Completes required training within prescribed timeframes
  17. Successfully participates in the DMH approved ACTT training
  18. Other duties as assigned

Medical Support Coordination Functions:

  1. Onsite team visit with each ACT Team at least twice a year to provide support in consistently implementing medical practices within community standards of care, Easterseals UCP policy and guidelines, OSHA Standards, high fidelity implementation TMACT, and meet requirements from other regulatory agencies as indicated. Coordinator will share findings from team visits with medical director, clinical director, and/or other leadership designee.
  2. Participation in enhancing Easterseals UCP’s ACT Teams efforts with integrated healthcare including engagement with the Monthly CCNC Practice Transformation Meeting and Monthly UNC Wakebrook Primary Care Meeting.
  3. Provide consult and technical assistance for the ACT medical teams and team lead following team’s request.
  4. Onsite medical practices orientation for medical staff and team leads.
  5. Support medical staff in effective use of electronic health record and e-prescribe tools.

Supervisory Responsibility:

There will be no direct supervisory responsibilities.

Non-Essential Job Functions:

The Medical Support Coordinator/Psychiatric Nurse Practitioner participates in the following:

  1. Assuring best practice standards for ACTT services are met within the team
  2. The interviewing process of prospective nursing or other clinical staff on ACT Team
  3. ACTT trainings and teachings to team members
 
Position Requirements

Minimum Qualifications:

The Medical Support Coordinator must have the following:

  1. hold a Master's or higher degree in Nursing or related field with primary focus on Nursing;
  2. have successfully completed a graduate level nurse practitioner education program accredited by a national accrediting body; and
  3. provide documentation of certification by a national credentialing body
  4. has an unrestricted license to practice as a registered nurse in North Carolina and, when applicable, an unrestricted approval, registration or license as a nurse practitioner in another state, territory, or possession of the United States;
  5. has successfully completed a nurse practitioner education program as outlined in Rule .0805
  6. is certified as a nurse practitioner by a national credentialing body consistent with 21 NCAC 36 .0801(8);
  7. Is board eligible or certified in psychiatry; or
  8. Is ASAM- eligible or certified; and
  9. Holds a current, unrestricted DEA license and is in good standing with federal agency; and
  10. Has a current NPI number; and
  11. Has individual liability insurance coverage for practice; and
  12. Must be able to work on full-time basis depending on needs of the program and the number of individuals to be served.
