Case Managers needed for full-time, temporary position in Montgomery County.
The Care Manager will be responsible for working with the local community to engage individuals with opiate use disorders to facilitate ongoing treatment and provision of other needed services to engage them and remain engaged.
Identify, establish, and maintain relationships with the prospective community referral source. Provide ongoing education and consultation to community partners. Make initial contact with individuals in the community where they are present. Conduct on-site assessment. Facilitate initiation into opiate use disorder (OUD) treatment. Create and individualize support plans for each person assessed. Regularly and directly communicate with the individuals' direct care team. Motivate and encourage individuals with OUD to stay engaged in both physical health and behavioral health treatment.
Assess and support individuals to address psychosocial needs and to re-establish family community relationships. Collaborate with administrative staff regarding required outcome data.
