Description

The Senior Outreach Worker participates in the development, implementation, monitoring and evaluation of supports in the CCHC service area for seniors. The efforts of the Senior Outreach Worker are directed specifically towards promoting/enhancing the effective integration and quality of life of seniors by providing individual practical support, referral, advocacy and cultural interpretation.

Program-Related Activities

  • Building relationships with seniors.
  • Assists seniors in connecting with needed resources in the Ottawa area.
  • Provides referral, advocacy and cultural interpretation as needed.
  • Assist in the meeting the goals set out in the grant application
  • Acts as a client advocate on individual and systemic issues.
  • Assists seniors in the community to build support networks.
  • Assists with facilitation of groups for target populations, as requested.
  • Participates in the development, implementation, monitoring and evaluation of education, and health promotion programs and services for individuals, families and the community.
 
Position Requirements

Education

  • Bachelor’s degree from a recognized university in a relevant discipline or equivalent combination of some post-secondary education and significant experience working with an older adult/senior population.

Professional Experience

  • Three to five years full-time relevant experience in a community setting.
  • Experience and interest in one or more of the following areas: community work/development health, seniors’ health and program delivery.
  • Experience working with older adults/seniors.
  • Experience working with volunteers in a social services or health care environment.
  • Experience in group education and knowledge of adult learning principles and experience in facilitating groups.
  • Experience and interest in working with marginalized and diverse clientele, such as low-income, high-needs, newcomer, and a variety of multicultural communities.
  • Experience in health promotion including community development and connecting with community groups and association.
  • Experience with facilitating physical education sessions for seniors.
  • Experience in program development, implementation, monitoring, and evaluation.
  • Experience in a non-profit, community-based organization.

Key Competencies

  • Thorough knowledge and understanding of community resources and community-based health care, as well as issues affecting women and people affected by mental illness.
  • Excellent interpersonal and communication skills, oral and written.
  • Demonstrated ability to work in a multi-disciplinary team environment.
  • Ability to adapt to change and have the willingness to learn.
  • Knowledge of and experience with group dynamics and group facilitation approaches.
  • Demonstrated proficiency in the use of computers and various software applications, including Microsoft Office programs, client database software, and electronic record keeping in a client services setting.

Linguistic Profile

  • This position is not designated under Carlington’s French Language Services Plan.
  • Proficiency in English required.
  • Bilingualism (French / English) is preferred, a third language relevant to our client population is an asset.

Other

  • Availability to work days, evenings and occasional weekends if required.
  • Use of a vehicle, valid driver’s license required.
  • Police Record Check for working with the vulnerable sector is required. 
  • CPR Certification is considered an asset.
 
Salary Range $25.33/hour - $29.79/hour
