Community First Health Centers is currently looking for a Registered Nurse - RN to join our team at our New Haven medical facility under our Maternal Infant Health Program (MIHP).

The ideal candidate for this role is a Registered Nurse who enjoys working with our medicaid population to provide services through home, onsite and telehealth visits.  Under the supervision of the MIHP Program Manager, you'll promote healthy pregnancies, positive birth outcomes and healthy infant growth and development.  This position will be located out of our New Haven, MI medical facility located at 58144 Gratiot Avenue but work will be conducted regularly in the field through client home visits.

 

 

Daily responsibilities of our Registered Nurse - RN may include:

  • Provide nursing service through home, office and/or telehealth visit based on MIHP plan of care
  • Maintain patient caseload, all charting and chart reviews
  • Perform initial assessments, development, stress and/or depression screenings
  • Weigh and measure infants; plot details on growth grid
  • Implement the MIHP care plans
  • Consult and/or coordinate with MIHP team and patients' medical provider for coordination of care
  • Provide follow up services and/or referrals based on patients' need
  • Serve as patient advocate; provide helath education as needed/required
  • Electronically enter all accurate patient information into a variety of systems
  • Complete all forms and billing information timely and accurately to meet all federal, state, insurance and Agency standards
  • Other duties as assigned

 

 

 

This is a full time; 40 hours per week opportunity.

Hours will be gained predominately Monday thru Friday; 830am - 5pm but some flexibility is needed based on client visits.

Position Requirements

You would be a great fit for Community First Health Center if you have:

  • Certificate of completion of a Registered Nurse (RN) program from an accredited college of nursing required
  • Current, unencumbered State of Michigan licensure as a RN
  • Ability to effectively manage multiple tasks, activities and responsibilities
  • Superior interpersonal skills with patients and coworkers
  • Reliable transportation with adequate insurance to utilize for client visits
