GENERAL DESCRIPTION:

Serves as a liaison between providers, patients, outside providers and insurance companies.

JOB RESPONSIBILITIES:

  • Interacts with patients, providers, and personnel on the phone and face-to-face
  • Processes patient referrals to specialists and instructs patients on the preparation needed for specific testing or procedures
  • Keeps patients informed of the status of the referral and specialist information
  • Under general supervision, generates and processes referrals to outside providers with respect to established policy and procedures in an accurate and timely manner
  • Provides information and assistance to referring physicians and facilities
  • Collaborates with physicians concerning patient placement and procedures
  • Tracks outstanding referral orders and provides follow-up to make sure results are received or if not received, documents in chart the reason results are missing
  • Participates in the Primary Care Health Home program as a Care Coordinator. Duties include:
    • Track referrals
    • Provide training and technical assistance
    • Manage and report data (can be separated into second part-time function)
    • Coordinate appointment scheduling for primary care health home team and enrollees
    • Remind enrollees regarding keeping appointments
    • Request and send medical records for care coordination
  • Monitors patient attendance at scheduled appointments and if appointments are missed, works with the patient to determine reason. Reschedules missed appointments if possible
  • Utilizes in-house database, internet and other sources to locate referral physicians and facilities appropriate for care needed
  • Obtains all pre-certifications as needed for referral services or other applicable orders
  • Works collaboratively with physicians and their office staff to expedite the admission or transfer of patients
  • Performs duties of medical records custodian as detailed in policy on release of medical records
  • Administers the reproduction of all patient records for transferring purposes
  • Works in close proximity with customer service representatives and non-SMCHC staff
  • Provides a high level of professional customer service
  • Maintains patient confidentiality
  • Maintains a clean, safe and organized work area
  • Keeps updated on all pertinent information affecting job and patient care
  • Handles reports, correspondence, etc, when requested by physicians and staff
  • Participates in required meetings for communication process
  • Performs other duties as assigned

 
Position Requirements

MINIMUM QUALIFICATIONS:

  • High school diploma or equivalent
  • At least two (2) years’ experience working with patients and the public.
  • Good interpersonal, written and verbal communication skills.
  • A means of transportation that would facilitate travel between clinics and to out of town meetings and training when it is deemed necessary and appropriate. Travel may be required with or without advance notice.
