Habilitation Specialist - ID/DD

Description

Duties include evaluation of each individual’s strengths, preferences, and needs by direct assessment and consultation with the individual and family members; Development of a service plan (ISP) that establishes measurable objectives and precise training strategies for attaining those objectives for assigned individuals; Monitoring of individual’s progress toward goals and objectives; Participation in periodic ISP reviews for programs; Monitor the quality of the individual’s work and skill building, and take appropriate steps to ensure goals are being met; Maintain complete, accurate and current program records, including daily notes and person centered plans; Check quarterly reviews for quality and accuracy; Ensure the facility and any program-owned equipment remains clean, sanitary and in good working order, as requested by supervisor; Assist in the administration of medications, replenishment of medications, repackaging of medications, and documentation of same; Assist with training and orientation of new employees, as requested by supervisor; Interact as a liaison with case managers and other care providers to insure program planning and quality of life that is in the best interest of the individual, as requested by supervisor; Assist in dealing with verbally and physically aggressive individuals, using agency approved techniques; Provide constant supervision of the individuals at all times (work, lunch, breaks, recreational activities, etc.); Transport individuals to and from program services and community activities.

 
Position Requirements

Associate Degree in human services field preferred. High School diploma and related work experience is acceptable in lieu of Associates degree. Basic knowledge of human services. Good oral and written communication skills. Effective problem solving and decision-making skills. Ability to understand the special training needs of intellectually/developmentally disabled individuals. Ability to successfully complete training in, and demonstrate practical use of, Medication Administration, Medication Repackaging, Medicaid Waiver training, CPR, First Aid, behavioral intervention training, etc. Ability to lift forty (40) pounds, with or without reasonable accommodation. Ability to work with and assist individuals with physical aggression and behavior issues. Current valid driver's license and good driving record. Willingness and ability to travel and use personal transportation in a work capacity. Willingness to work a flexible schedule as needed.

About the Organization CMCSB was formed in 1972 as a Chapter 10 Community Services Board under Virginia law. CMCSB provides mental health, intellectual disabilities and substance abuse treatment services to the residents of Buchanan, Russell, and Tazewell Counties. At the present time, CMCSB operates over twenty locations, to include outpatient offices, mental health recovery programs, group homes and day support programs for intellectually disabled individuals, and a residential alcohol and drug treatment program. In addition, CMCSB has several services that are provided directly in an individual's home or in a supported environment. CMCSB also provides a 24-hour crisis team to assist community agencies and individuals in crisis situations. CMCSB employs approximately 500 employees, which include full-time and part-time employees, as well as individuals receiving services who are employed to work in various day programs.
