POSITION SUMMARY
The Integrated Health Home Peer Support Specialist's primary responsibility is to encourage clients to take control over their own health and wellness and promote the concept of 'whole-health.' The Peer Support Specialist is responsible for providing support services and serving as an advocate. The Peer Support Specialist performs a wide range of tasks to assist clients in their own recovery process and role model competency in ongoing coping skills.

ESSENTIAL FUNCTIONS

  • Manages/plans operations of WMHC's drop-in Peer Support Program ("The Hope Center").
  • Utilizes goal-setting processes to further consumers' recovery process. Develop additional programs/recovery groups to meet the needs of the members.
  • Assists consumers in identifying personal goals for recovery and determines interventions based on these goals.
  • Provides resources and referrals to community and social support services.
  • Promotes substance abuse prevention, smoking prevention and cessation, obesity reduction, and increased physical activity.
  • Leads as well as teaches consumers how to facilitate recovery dialogues and/or self-management plans so that members can attain personal health goals.
  • Assists consumers in creating a Wellness Recovery Action Plan (WRAP).
  • Utilizes and teaches problem-solving techniques with individuals and groups.
  • Teaches consumers how to identify and combat negative self-talk and teaches them how to identify and overcome fears.
  • Provides and advocates for effective recovery-based services, and reduces barriers to access to care.
  • Teaches and role models the value of every individual's recovery experience.
  • Reviews patient profiles with IHH Health Team to assess needs of consumers.
  • Establishes and maintains routine collaborative meetings with system partners to achieve benchmark standards and improve patient care.
  • Collaborates with Health Home Team and treating providers to develop and implement care coordination plans.
  • Demonstrates flexibility in job duties and schedule in order to aid in better serving the organization, the consumers, and to assist reaching benchmark standards.
  • Allows for flexibility in scheduling to accommodate crisis situations for patients.
 
Position Requirements

REQUIRED EDUCATION/EXPERIENCE

  • High school diploma required/bachelor’s degree preferred
  • 2 years experience in mental health/human services field
  • Certified Peer Support Specialist, or eligible to become certificatied within first 6 months of employment
  • Iowa driver's license w/ good driving record


PHYSICAL DEMANDS
This position requires the ability to remain in a stationary position for extended periods of time, move about inside the office to access file cabinets/office machinery/et cetera, operate basic office equipment such as computers/calculators/copy machines/printers/scanners, communicate verbally/in writing, and lift up to 75lbs as needed.
