Clinical LPN 

Description

Serve as a Clinical LPN for Waubonsie Mental Health Center in Outreach Clinics. Provide Tele-Psych service using videoconferencing equipment to connect with a mental health prescriber to facilitate a face to face clinic visit to serve clients who need assistance in medication management.

Job Duties

Demonstrates competency in knowledge and skills of video and electronic equipment set-up in preparation for video conference

Utilization of Electronic Health Records (EHR)

Perform clinic nurse functions such as: collect and record vitals, reconcile medication list, review allergy list, gather preliminary information such as scores from check in assessments (PHQ9, MDQ, ACE, GAD7), room the client and give a verbal hand-off to the prescriber, put in orders for labs, collect DNA samples for genetic testing, administer injections as ordered by prescriber, process for any internal or external referrals of clients, printing of any prescriber recommended teaching materials, gathering medication samples, review incoming correspondence with prescriber (ex: labs, genetic reports, medical records, pharmacy communications), check phone messages and email, follow up with client if any further teaching need reviewed, facilitate follow-up appointment date and length of visit with support staff for scheduling

Telephone triage to determine priority of client care, assess the patient, determine whether they need to be seen and obtain care for them, and determine that the patient should receive emergency treatment or that they should be transferred to a hospital. Demonstrate flexibility, adaptability, priority setting, problem solving, and organizational skills, reliability and maintains confidentiality

Travel to various location in Montgomery and Page Counties.

Position Requirements

Education/Experience:

Graduate of an accredited program for Licensed Practical Nurses

Current license to practice nursing in the State of Iowa for Licensed Practical Nurse

Previous clinical experience in a medical office and behavioral health preferred

Knowledge/Skills:

Computer data entry experience

Mandatory Reporter certification

Strong interpersonal skills

Strong computer skills

Ability to work as a team member

Ability to understand and apply guidelines, policies and procedures

Strong verbal and written communication skills

Other Requirement:

Reliable Vehicle to get you around Southwest Iowa

Valid Drivers’ License, with good driving record

Valid Auto Insurance

Extensive Background Check, also Dependent Adult and Child Abuse registries, Sex Offender registry, and Criminal History Check
