EAP Care Managers & Assistants - REMOTE Opportunities

Description

Employee Assistance Care Manager needed for full-time, temporary position working remotely from 2:30-11pm.

Essential Functions:

1. Receive and respond to intake calls including:

  • Explains EAP services and confidentiality.
  • Obtains and records intake case information in Care Management System
  • Risk screening and facilitate connection of call to licensed clinical staff and emergency services as appropriate and directed.
  • Provides referral services to EAP clients.

2. Provides delivery of services to EAP clients including:

  • Provides information and helps member access services.
  • Provides members with appropriate referral to EAP affiliate.
  • Reviews non routine level of care with supervisor or Senior Care Manager
  • Facilitates referral process and contacts service providers or subcontracted EAP affiliates.
  • Follow up with client and/or service provider/ affiliate as required per Standard Operating Procedure.
  • Documents all contacts with clients and pertinent others accurately, completely, and concisely.
  • Facilitates connection or referral to other available services such as legal/financial consultation, work-life resources, or other solutions that may be available to Health Advocate members
  • Provide other Employee Assistance services as assigned including case management of special needs.
  • Provide telephonic services in accordance with setting's standard procedures
  • Collaborates with Care Managers and others to ensure compliance with SOP, to provide excellent member service and experience, and to support professional operations and environment within the EA division.
  • Obtain prompt supervision and consultation when risk screening is positive, or the level of care may be other than routine for any reason.
  • Develop and maintain familiarity with setting's EAP Standard Operating Procedures as demonstrated by the delivery of service and performance of duties.
  • Maintain reliable attendance and timely arrival being ready to work as scheduled.
  • Miscellaneous duties as assigned by Supervisor or Executive management
 
Position Requirements

MSW or LSW with a minimum two years of experience providing service in a health or human service setting including call handling, risk screening and case management. A combination of paid and volunteer experience including outreach, intake and assessment, telephone support or hotline, Employee Assistance or similar call center, or direct service in a community treatment or recovery center will be considered.
