Case Manager - Crisis/Outreach (CCBHC) 


Wyandot BHN is seeking a dedicated Crisis Case Manager/Outreach Specialist to join our CCBHC adult services team! The Crisis Case Manager/Outreach Specialist provides short-term community based outreaching and engagement services to adult consumers with a mental illness, co-occurring substance use disorder and/or have been impacted by COVID-19.  Once the immediate crisis is resolved and/or the person has established care with other service providers, the outreach specialist will provide a “warm hand off” to longer term service providers to ensure continuity of care. 

The CCBHC model (Certified Community Behavioral Health Clinic) provides open access to care for clients through a comprehensive range of mental health & substance use disorder services. The CCBHC model emphasizes 24-hour crisis care, evidence-based practices, care coordination with local primary care/hospital partners & integration with physical health care. Essential Functions:

  • Works with team members (specifically the mobile crisis responder), community partners, and service organizations to provide direct-service to  adults who have been identified to have been impacted by COVID-19 and/or need immediate, crisis services.  Direct services may include, but are not limited to: crisis intervention, assessment, short term crisis counseling, assist with connection to available resources, housing, employment  and system navigation skills.
  • Utilize intentional outreach strategies throughout the community to connect individuals to services by reducing external barriers.
  • Follow-up with consumers upon discharge from the hospital or crisis stabilization centers.
  • Monitors program data and develops monthly reports.
  • Completes timely and accurate documentation of services in electronic medical record.
  • Actively participates in the research, development and implementation of emerging multi-system models and practices.
  • Coordinates community / mobile emergency responses in conjunction with the Crisis & Access Services team as needed.
  • Coordinates with 24/7 crisis line contractor as needed.
  • Confers with various Center staff to ensure optimum consumer care.
  • Models and inspires flexibility and creativity in the delivery of clinical services.
  • Assures staff and program compliance with Medicaid, State licensing regulations, and other contractual standards.
  • Respond to individuals during crisis situations as needed. 
  • Follow-up on referrals from family, friends, co-workers, and other professionals. 
  • Help individuals determine eligibility for Medicaid, Medicare, etc. 
Position Requirements

Job Requirements:

  • Bachelor’s degree from an accredited University, with preferred experience in the human service and/or criminal justice fields.  Equivalent experience may be substituted in lieu of degree. 
  • Valid driver's license, vehicle insurance & independent transportation.
  • Preferred experience in the mental health field, specifically with community based and/or outreach work
  • Excellent interpersonal and communication skills, including both oral and written.
  • Ability to establish effective working relationships with both Center staff and various community agencies. 
  • Ability to work autonomously in the community and be proactive in outreach
  • Ability to facilitate planning meetings with individuals from multiple systems. 
  • Proficient in use of technology, including Microsoft Office. 
  • Provide and maintain dependable transportation and auto insurance. 
  • Ability to build and maintain close working relationships with clients and community partners. 
  • Complies with all relevant Center policies and procedures. 
  • Performs other related duties as deemed appropriate by immediate supervisor or designate. 
  • Ability to recognize and be sensitive to cultural and ethnic differences. 
  • Must be able to safely perform essential job functions, with or without reasonable accommodations.
