Turning Point Community Programs is seeking a Housing Coordinator for our Coloma Center Program in Roseville. Turning Point Community Programs (TPCP) provides integrated, cost-effective mental health services, employment and housing for adults, children and their families that promote recovery, independence and self-sufficiency. GENERAL PURPOSE

Under the administrative supervision of the Clinical Director or Program Director, this position is responsible for the attainment of housing resources for clients and provides direct assistance when needed.

DISTINGUISHING CHARACTERISTICS

This is an at-will direct service position within a program. The position is responsible for the attainment of housing resources and assisting clients with securing supportive housing placements. Additional support in areas of psychosocial, rehabilitation, and advocacy will be provided as needed.

ESSENTIAL DUTIES AND RESPONSIBILITIES – (ILLUSTRATIVE ONLY)

  • Works with clients to obtain and keep housing of their choice.
  • Develops and maintains a listing of housing options for clients.
  • Safely transports members and other passengers to and from appointments and activities using own personal vehicle.
  • Supports with completing and submitting necessary documentation to secure housing for clients.
  • Attends in-services and training on housing in the community.
  • Acts as liaison with board and care operators, room and board operators, county, and staff.
  • Complete SOAR Certification Training. When applicable, assist clients with SOAR application to support attainment of SSI benefits to secure and maintain housing.
  • May occasionally carry a small caseload as determined by program needs
  • Keeps all other involved staff and agencies informed of client’s progress.
  • Provides documentation in the charts that is consistent with the service plan.
  • Provides prompt intervention in the event of a crisis both to stop the crisis and to notify, when indicated, persons and agencies necessary for the resolution of the crisis.
  • Provides “on-the-spot” counseling that is both helpful to the members and consistent with the philosophy of the program.
  • Attends to the safety, health, and wellbeing of clients.
  • Completes paperwork as assigned in a timely manner.
  • Provides support to other staff members as needed.

Knowledge Of:

  • Principles and goals of community mental health.
  • Principles and goals of the “self-help model.”
  • Principles and goals of the “consumer-driven model.”
  • Principles and goals of the “harm reduction model.”
  • Psychosocial rehabilitation’s treatment and programming.
  • Board and care licensing requirements and procedures.
  • Community resources, including housing resources

Ability To:

  • Work and communicate with staff, clients, families, community agencies and Professionals.
  • Perform crisis intervention strategies.
  • Work effectively under stress and conflict.
  • Exercise appropriate judgment and decision-making.
  • Be flexible and adaptable in any given situation. 
  • Work as a member of a team.
  • Work under time deadlines and pressures.
  • Work with minimal supervision.

Schedule: Monday - Friday 8:30 am - 5:00 pm

 
Position Requirements

MINIMUM QUALIFICATIONS AND CERTIFICATIONS & LICENSES

Education, Training and Experience:

A typical way of obtaining the knowledge, skills and abilities outlined above is through graduation from a four (4) year college or university with Bachelor’s Degree in Social Work or related field and four years varied experience as a provider of mental health services or Associate Arts Degree and six years of full time/equivalent (FTE) direct care experience in a behavioral health setting. At least two of the six years must be post AA experience.

Experience in Housing Support Services Preferred.

Licenses; Certificates; Special Requirements:

  • California driver’s license & current vehicle insurance/registration if driving; and,
  • Reliable means of transportation capable of passing vehicle safety inspection if more than five years old excluding all modes of two-wheeled transport inclusive of bicycles, mopeds and motorcycles
