Overview

Under the direct or indirect supervision of a Registered Nurse responsible for providing professional skilled nursing services according to established Policies and Procedures. Responsible for ensuring continuity of care by providing direct care and supervising CNAs, as well as coordinating care with other support personnel.

 


Duties and Responsibilities

  • Monitor / provide total resident care
  • Participate in the development of an individualized interdisciplinary plan of care for each resident in assigned group on a monthly basis
  • Implement plan of care for the resident based on assessments, written care plan and goals established by the Inter-disciplinary Care Team
  • Assess the resident's condition and nursing needs and implement appropriate action throughout entire shift
  • Monitor resident outcomes, take action according to licensed scope of practice and report findings to RN as required by policy, regulation or when outcome requires intervention beyond licensed scope of practice
  • Document resident status in medical record including progress notes, communication logs and care plans in a clear and pertinent manner within the shift in which status changes are noted
  • Notify MD of changes in resident's condition and completely carry out physician orders accurately and in compliance with time frame indicated on orders, care plan or dictated by resident needs
  • Give thorough report at change of shift by performing walking rounds and documenting on calendars and communication logs
  • Perform resident admissions and discharges according to Santa Teresita procedures
  • Perform technical and manual procedures including the administration of medication and clinical treatments according to physician orders and established standards set by Santa Teresita policy and quality practices within LVN scope of practice
  • Participate in Performance Improvement program as requested
  • Participate in orientation of new personnel and assist in performance evaluations when requested
  • Instruct and supervise CNAs in order to accomplish the plan of care which has been developed for each resident
  • Respond to emergency situations according to policy and procedure
  • Serve residents in a manner conducive to their safety, dignity and comfort
  • Develop appropriate relationships with residents and families in which there is an exchange of mutual esteem and caring within professional boundaries
  • Educate residents and their support group and include them in both development and implementation of plan of care
  • Contribute through positive attitude, vigilance in work habits and ready cooperation to the overall environment in which residents and families experience their well-being as the center of all nursing efforts
  • Follow proper body mechanics and preventive safety measures at all times
  • Follow proper infection control techniques to prevent cross contamination of resident care areas
 
Position Requirements

Qualifications and Education Requirements

  • Current licensure from the California Board of Vocational Nurse and Psychiatric Technician Examiners
  • Current CPR Certificate and Basic Cardiac Life Support Certificate
  • Current in IV Therapy Certificate within three (3) months of hire
  • Able to read, write and speak English fluently in a manner that can be clearly understood by others
  • Complete 30 hours of continuing education every two years (minimum of 10 units in any given year)

  • Self-motivated in time management and prioritization of multiple residents needs and/or tasks in order to organize workload so that all functions are completed appropriately
  • In-depth understanding of the aging process, its physical, psycho-social and spiritual consequences and the ability to apply that understanding in practical support of each individual resident
  • Attends all in-services and other training sessions required by this position
