Purpose of Your Job Position:
The primary purpose of your job is to provide direct nursing care to the residents, and to supervise the day-to-day nursing activities performed by resident care assistants. Such supervision must be in accordance with current federal, state, and local standards, guidelines, and regulations that govern our facility, and as may be required by the Director of Nursing Services or Resident Care Supervisor to ensure that the highest degree of quality care is maintained at all times.
Duties and Responsibilities
Monitor / provide total resident care
Participate in the development of an individualized interdisciplinary plan of care for each resident in assigned group on a monthly basis
Implement plan of care for the resident based on assessments, written care plan and goals established by the Inter-disciplinary Care Team
Assess the resident's condition and nursing needs and implement appropriate action throughout entire shift
Monitor resident outcomes, take action according to licensed scope of practice and report findings to RN as required by policy, regulation or when outcome requires intervention beyond licensed scope of practice
Document resident status in medical record including progress notes, communication logs and care plans in a clear and pertinent manner within the shift in which status changes are noted
Notify MD of changes in resident's condition and completely carry out physician orders accurately and in compliance with time frame indicated on orders, care plan or dictated by resident needs
