Turning Point Community Programs is hiring a Case Manager/Mental Health Worker (formerly known as: Personal Service Coordinator III) at our Providence Center Program working as a member of The Nevada County Behavioral Health's interdisciplinary team. GENERAL PURPOSE 

Under the administrative supervision of the Team Leader or Program Director, this position is responsible for providing psychosocial, rehabilitation and recovery-oriented services for adults.

DISTINGUISHING CHARACTERISTICS

This is an at-will direct service position within a program. The position is responsible for assisting and advocating for our clients in meeting their expressed goals while living in the community.  Additional support in areas of medication management, housing, counseling and advocacy will be provided as needed.

ESSENTIAL DUTIES AND RESPONSIBILITIES – (ILLUSTRATIVE ONLY)

  • Works as a member of Nevada County Behavioral Health interdisciplinary team assisting clients.
  • Supporting clients with developing practical housing skills and positive relationships with peers, housemates, neighbors and landlords; and cultivating strong relationships/networks with landlords and other housing service providers.
  • Provides consultation and assistance to landlords, property owners and employers.
  • Support clients daily for one week or more if needed when moving into a BH SIL.
  • Provides prompt intervention in the event of a crisis both to stop the crisis and to notify, when indicated, persons and agencies necessary for the resolution of the crisis.
  • Provides “on-the-spot” counseling that is both helpful to the clients and consistent with the philosophy of the program. 
  • Provides rehabilitative services that focus on recovery and wellness including: money management and personal hygiene.
  • Consults with, and meets regularly, with participant’s family, team members, or other significant persons with the participant’s consent.
  • Keeps all other involved staff and agencies informed of client’s progress.
  • Teaches wellness skills and assists IST participants in practicing healthy lifestyles (e.g., adequate nutrition, cooking, yoga, relaxation, and healthy activities).
  • Coordinates services and assists participants in completing all required forms and information, including evaluation data, health history, financial information, and other periodic measures of health and wellness.
  • Assists IST members in preparing, administering, and implementing Integrated Health Care Plans.
  • Links participants to additional community services as needed, including: SNMH, Hospitality House, Probation and other county agencies for those in need of forensic support.
  • Assists in establishing participant’s eligibility for Medi-Cal or other benefits and advocates for continuation of benefits, when appropriate.
  • Advocates for participants in all areas of services and empower them to achieve their chosen goals.
  • Safely transports clients to appointments using own vehicle or Turning Point program vehicle.
  • Insures the safety, health, and well-being of clients. Provides support to other staff members as needed.
  • Carries out a variety of treatment or special project activities under the direction of professional staff.
  • Provides documentation in the charts that is consistent with the service plan.
  • Completes paperwork as assigned in a timely manner.
  • Is personally responsible and held accountable for work hours and time management.
  • Attends in-services and training on housing in the community and also attends all team and agency staff meetings unless prior approval for non-attendance is obtained.
  • Check TPCP e-mail at least twice a week for important updates and notices.

Schedule: Monday - Friday 8:00 am - 4:30 pm

 
Position Requirements

MINIMUM QUALIFICATIONS AND CERTIFICATIONS & LICENSES

Education, Training and Experience:

A typical way of obtaining the knowledge, skills and abilities outlined above is through graduation from a four (4) year college or university with Bachelor’s Degree in Social Work or related field and four years varied experience as a provider of mental health services or Associate Arts Degree and six years of full time/equivalent (FTE) direct care experience in a behavioral health setting. At least two of the six years must be post AA experience.

Licenses; Certificates; Special Requirements:

  • California driver’s license & current vehicle insurance/registration if driving; and,
  • Reliable means of transportation capable of passing vehicle safety inspection if more than five years old excluding all modes of two-wheeled transport inclusive of bicycles, mopeds and motorcycles
