Description

SUMMARY: Integrated, comprehensive primary care is increasingly focused on the combination of prevention, continuity, education and health maintenance with the support of various enabling services. The Case Manager for Pediatrics is an important role in reaching out to and maintaining quality services to vulnerable and underserved children. The Case Manager for Pediatrics will act as a patient advocate and guide within the context of health center operations and the health goals set by the family in collaboration with primary medical and behavioral health care providers. The Case Manager for Pediatrics will assist families navigate health care access as well as access to other available services and benefits, including community and governmental programs. The Case Manager for Pediatrics will also monitor high-risk for loss to care children, working with families and guardians to maintain treatment engagement and compliance.

 
Position Requirements

ESSENTIAL DUTIES AND RESPONSIBILITIES: The essential duties and responsibilities are conducted within the context of strong interpersonal, verbal and written communication, with attention to detail and sense of collaboration with the primary health care team, including medical, dental and behavioral health services as well as external resources as needed

  • Coordinates with pediatric patients and their families (natural or legal), upon referral from a provider, to develop, implement and monitor strategies to effectively engage in recommended treatment plans and to identify and access additional external resources in support of treatment plans;
  • Promotes and facilitates effective communication among patients, families, primary care providers and other internal and external service providers;
  • Informs health care team of patient and family needs and barriers; strategizes with health care team to best overcome such barriers;
  • Maintains knowledge of and effective relationships with community resources specific to children and families; explains resources and benefit options to families and assists families in navigating application processes;
  • For the most part, provides short-term interventions and assistance, with assistance and referral to community resources for longer-term solutions; works with health care team to reinforce treatment plans and health education activities;
  • Monitors pediatric patients identified as high-risk for loss to treatment and works to engage patients and families in treatment plans;
  • Documents services provided to pediatric patients and families; maintains and submits reports in a timely manner and within appropriate timeframes;
  • Reviews system problems and difficult cases with supervisor on a regular basis;
  • Contributes positively to the efficiency, accessibility, productivity, quality, safety, compassion and professionalism of the work setting;
  • Adheres to ODCHC’s policies and procedures; and,
  • Completes other duties and responsibilities as identified and assigned by the Health Resources Manager.

 

QUALIFICATIONS AND EXPECTATIONS: The successful candidate will demonstrate an attitude and commitment to creating positive and effective patient experiences, facilitating patient care and working efficiently and collaboratively with families and other members of the health care team.

  • Bachelor’s degree in social service or health related field and two years of experience in a similar role; or an equivalent combination of education and experience;
  • Experience working with children and families experiencing poverty, behavioral health issues, disabilities, health concerns and/or social health challenges;
  • Ability to work with children and families with diverse experiences, including diverse economic, educational and cultural backgrounds;
  • Ability to work as a member of the health care and enabling services team, communication pertinent patient and family information to the team and support medical and behavioral health provider decisions;
  • Demonstrated flexibility and creativity in assisting and supporting patients and families in attaining desired outcomes;
  • Excellent interpersonal skills, both written and verbal;
  • Ability and desire to help patients understand follow-up instructions, patient information handouts, prevention activities and self-management activities; and to clarify preparations for upcoming procedures;
  • Accurate and rapid typing; comfortable in computer applications, including Word, Outlook and EPIC (preferred); ability to write documentation as well as routine reports and correspondence;
  • Organizational skills with focus on tracking patient care and improving patient experiences; knowledge of ODCHC referral process and follow-up; knowledge of internal and external resources and benefit programs;
  • Ability to handle difficult or confrontational situations in a calm, consistent and equitable manner;
  • Valid California driver’s license, insurable driving record and reliable transportation.

SUPERVISORY RESPONSIBILITIES: None.

SUPERVISION AND SUPPORT: The Case Manager for Pediatrics reports to the Health Resources Manager. The Case Manager for Pediatrics is an integral part of the health care team and may work in multiple clinics, with multiple medical and behavioral health clinicians.

PHYSICAL REQUIREMENTS: This is a clinic-based position. The physical requirements described are representative of those needed to successfully perform the essential duties of the position. Reasonable accommodation will be made to allow otherwise qualified candidates to perform these functions.

  • Speaking and hearing sufficient to communicate effectively by telephone, video or in-person at normal volumes;
  • Vision adequate to read documents, computer screens, forms, designs and differentiate colors as necessary;
  • Ability to use standard computer equipment; ability to use keyboard and view computer screens for extended periods of time;
  • Ability to lift and carry packages and boxes weighing up to 25 pounds;
  • Ability to sit, stand and move around the clinic for extended periods of time; and,
  • Ability to travel among clinic and administrative sites on a regular basis.
