This is a temporary or temp-to-perm position providing full-time coverage (8am-5pm) for an internal medicine team at a Kansas City area hospital. Missouri licensure (LMSW or LCSW) and previous acute medical experience is required. Social workers will be responsible for: Developing a comprehensive psychosocial assessment to age appropriate patient/family needs and concerns; Coordinating post-hospitalization services including home care, nursing home placement, durable medical equipment, financial assistance, counseling and other community resources; Collaborating and communicating with all members of the treatment team; Documenting all activities in the electronic medical record   Position Requirements Candidates must have a masters degree in social work from an accredited school of social work and be licensed in Missouri as a LMSW or LCSW.
