Description

POSITION SUMMARY

The pharmacy Medication Reconciliation Pharmacy Technician, working under the supervision and instruction of a licensed pharmacist, performs procedures and duties relating to the service’s Scope of Care. This can include collecting patient’s medications histories accurately, as well as serving operational pharmacy technician roles in the main pharmacy when needed including: preparation of both sterile and non-sterile medication dosage forms if applicable, automated dispensing functions, special project assignments, and prioritizing work responsibilities. For successful job performance, the pharmacy technician should be flexible and show keen attention to detail, exhibit initiative and be willing to work as part of a team. In addition, this individual should be cooperative and willing to cross train to fill in for other team members as necessary. The technician must be able to provide services as appropriate (under the direct supervision of a pharmacist) to meet the needs of the patient population at Tuba City Regional Health Care Corporation.

ESSENTIAL FUNCTIONS:

  1. Permissible activities of a pharmacy technician acting in compliance with all applicable statutes, rules and under the supervision of a pharmacist
  2. Ability to navigate the hospital and select patients for review.
  3. Ability to navigate the electronic health database to review medication list, enter a new medication history or update an existing one.
  4. Ability to communicate with patients of all education levels.
  5. Communicate effectively and collaborate within the hospital.
  6. Collaborates with charge nurse and patient nurse to determine when patient can be interviewed.
  7. Uses good clinical judgment skills to determine accuracy of medication history list as obtained. Solicits other sources of information to verify accuracy.
  8. Establish rapport with patient to obtain medication history, determine if patient or family member can provide medication history, or call patient's pharmacy or physician's office for medication history.
  9. Interviews patients to obtain or verify allergies and medication histories including prescribed medications, over-the-counter medications, and dietary and herbal supplements
  10. Prompts patient for additional medications, by specifically suggesting over the counter (OTCs) medications including herbals, vitamins, pain relievers, cold products, patches, lotions & creams, eye drops, sprays, and inhalers. Determines if patient was on recent antibiotics prior to admission.
  11. Assess incompleteness of current list of medications looking at drug, dose, frequency and indication for use.
  12. Determines time last dose of each medication was taken prior to admission for future scheduling, determines compliance of medication consumption prior to admission, and obtains indication for PRN medications and actual frequency of use of PRN medications.
  13. Promptly identifies any medication history issues with pharmacist and help identify and facilitate correction of medication discrepancies
  14. Has excellent knowledge of brand and generic medication names, common dosages and dosage forms and frequency of administration of each medication.
  15. Contributes to the safe and efficient operation of the Pharmacy Department.
  16. Takes initiative in doing miscellaneous functions within the department.
  17. Maintains responsible length of time for breaks.
  18. Demonstrates a positive attitude regarding telephone usage.
  19. Volunteers for projects and willingly assists co-workers to complete daily responsibilities. Works as a team player and assists where needed.
  20. Unit Inspection Functions.
  21. Operational Pharmacy Technician functions (including automated dispensing cabinets, medication transport, medication credits, compounding).
  22. Perform as an active member of the pharmacy department team.
  23. Ensure proper PPE is worn at all times while on duty including but not limited to, face mask, gloves, gown, isolation gown, NIOSH-approved N95 filtering facepiece respirator or higher, if available), and eye or face shield.
  24. Complete all donning and doffing tasks in a safe acceptable method and discard of used PPE accordingly. (see CDC website for most current updates)
  25. Complete task training for all routine cleaning and decontamination processes for all surfaces contaminated by a communicable disease to ensure a high level of patient, visitor, employee and external customer satisfaction
  26. Completes all electronic health record entries accurately and timely pertinent to patient care role.
  27. Participates in departmental workflow and or testing teams as related to electronic health record or other project initiatives.
  28. Performs all other duties as assigned by supervisory Pharmacist.

Physical:

Standing for prolonged periods, stooping, bending, reaching, walking and carrying drugs, moderate lifting of totes of drugs, charts, and boxes weighing up to 30 pounds.

Mental:

Immediate response to telephone and verbal requests. Mental attention and assertiveness are required at all times. The ability and willingness to learn new information is an expectation.

 
Position Requirements

NECESSARY QUALIFICATIONS

Education:

  • High school diploma or GED; Completion of Certified Pharmacy Technician Program

Certification:

  • Current valid pharmacy Technician License through a Board of Pharmacy. Upon joining TCRHCC the candidate must obtain a medication history technician certification within nine months of hire. Failure to obtain certification within nine months will result in termination of employment at TCRHCC.
  • Maintain technician licensure and credentials in good standing and maintain continuing education credits as required by Pharmacy Technician Certification Board, the Board of Pharmacy, and any other regulatory agencies.

Experience:

  • Must demonstrate skills in the practical use of electronic systems, ability to navigate the electronic medical record, intra-venous preparation, and satisfactory sterile technique(s).

Other Skills and Abilities:

A record of satisfactory performance in all prior and current employment as evidenced by positive employment references from previous and current employers. All employment references must address and indicate success in each one of the following areas:

  • Positive working relationships with others
  • Possession of high ethical standards, no history of complaints and no prior records of substance abuse
  • Reliable and dependable; reports to work as scheduled without excessive absences
  • Electronic Medication cabinet experience
  • Articulation skills (writing and verbal)
