Completes intake and psycho-social assessment to assigned clients and their families in their place of residence Assist families with identifying community or agency resources Maintain knowledge of hospice philosophy, services, admission criteria, and so on Prepare and maintain accurate individualized patient care records Communicates all admission assessment findings to interdisciplinary team electronically   Position Requirements MSW, LMSW or LCSW-C required Three years post master’s experience preferred, with at least 1-3 years of experience with hospice or home care, nursing home, or rehabilitation centers Hospice, bereavement, or terminally ill patient experience preferred Current Driver’s License and reliable transportation   This position is currently not accepting applications.
