Social Worker needed for full-time, temporary position in Philadelphia County. Needed for 12 weeks beginning mid-April.
Conduct comprehensive biopsychosocial assessments for a diverse patient population. Complete psychosocial section of initial assessments within 30 days of assignment and conduct mandated outreach to facilitate completion of the initial assessment.
Develop, coordinate, and implement a client-centered multidisciplinary care plan that establishes SMART goals and includes risk reduction strategies as needed. Institute strategies to manage interventions and time-framed care-plan reviews to meet agency and accrediting entities' requirements. Provide clinical consultation to the treatment team. Secure and utilize information from treatment team to inform successful encounters and interventions.
Maintain communication that keeps treatment team informed of key needs and opportunities related to patients' healthcare. Document all interactions with and on behalf of clients in the EMR. Monitor all referrals and tasks in order to provide appropriate support to maximize successful completion.
