Salud Para La Gente (SALUD) provides high quality, comprehensive and cost-effective healthcare to underserved low-income communities in the Monterey Bay area, including Santa Cruz County and North Monterey County. We began in Watsonville 1978 as a storefront “free clinic,†and with the ever growing need for health services, and in 1992 became a federally qualified community health center. Today we are a primary health care network with 11 clinic sites and continue provide high quality services to patients of all ages.

We are seeking an experienced, organized and detail-oriented Comprehensive Prenatal Health Worker (CPHW) to coordinate and deliver patients' education and access to specialty care providers and services for their prenatal needs. Responsibilities include, but are not limited to, the following:

  • Perform client orientation into the CPSP Program.
  • Assist in educational classes and videos.
  • Perform patient assessments, reassessments, interventions and follow-up services in:Â Obstetrics, Nutrition, Psychosocial services and Health Education.
  • Acts as a community service liaison.
  • Assists with individual case coordination and case management.
  • Consults with licensed staff such as MD or NP for dispensing of Prenatal Vitamins.
  • Assists patients with forms such as WIC and Disability.
  • Leads Prenatal and Parenting Education, individual and group sessions.
  • Assists with referrals to outside organizations such as WIC, genetic screening and dental.
  • Perform patient CPSP orientation:Â services provided, OB staff, program adherence and qualifications.
  • Participates in Off-Site clinics as required.
  • Maintains CPR Certificate and a valid California driver's license with vehicle insurance.
  • Develops individual care plans and evaluates quality of care.
  • Performs other duties as assigned.

Position Requirements

MINIMUM QUALIFICATIONS

  • High school diploma or GED
  • CPSP Certificate or able to acquire within three (3) months
  • Knowledge of medical terminology in Spanish and English
  • Experience in case coordination and case management
  • Willingness to work with infected HIV clients
  • Current CPR certification
  • Fluent Bilingual: Spanish and English
  • Valid California' driver's license and current vehicle insurance

DESIRABLE QUALIFICATIONS

  • Six to twelve months' experience in CPSP preferred
  • Bicultural Spanish/English preferred

PHYSICAL REQUIREMENTS

  • Standing, walking, sitting, typing, reaching, bending, moving and/or lifting up to 25 pounds.

SALARY & BENEFITS

Salary: $19.18 to $24.04 per hour

Benefits: CA Sick Pay, prorated per hours worked
