Share Email Opening Title Patient Navigator  Post Internal Days 10   Description Comunity Health Care, Inc. is currently adding a new position for a Patient Navigator to work in our River Drive Clinic in Davenport, IA. Working within scope of practice and under the general supervision of the Population Health Manager, the Patient Navigator identifies patients with preventative and chronic condition care gaps and performs patient outreach to close gaps.
