Part-time Physical Therapy Assistant - Home Health

Position Requirements

As a Physical Therapist Assistant, you will:

• Provide therapy services consistent with the patient's Plan of Treatment.

• Observe patient during treatment, compile data and report patient's progress.

• Act as a liaison and maintain contact with patients, Physical Therapist and Clinical Team to ensure effective care coordination.

• Provide hands-on care and education of the patient.

• Works with other highly skilled clinicians through a multi or interdisciplinary team approach

Job Responsibilities:

• Travels to patient homes and provides physical therapy care and patient assessment duties. Implements treatment plan. Conducts treatments for exercise, posture correction, etc. Observes, records, and reports the patient’s reaction to treatment and changes in condition. -- Instructs patients, family members of patients and other health personnel in the use of equipment, or phases of therapy with which they may work with patient

• Ensures for appropriate discussion of patients and required completion of documentation.

• Coordinates discharge plan decisions and the final assessment with the Physical Therapist.

• Coordinates on-going care with Physical therapist, the RN Case Managers and other clinical staff to ensure coordination of care. Reports any concerns or issues. Identifies any barriers to achievement of identified goals/outcomes and works in collaboration with the RN Case Manager and other patient care team members to assure coordination of care is provided to attain desired outcome. 

• Communicates significant changes in patient condition. 

• Documents plan, notes, requirements per regulations in timely manner and accurate.

• Follows-up with RN or Director of nursing  to ensure notification and documentation of missed visits is provided in accordance with proper procedures in a timely manner.

• Completes and submits timesheets or time logs to supervisor on a timely basis which accurately document hours, mileage, travel and other time worked

• Attends and participates in mandatory meetings, in-services, and conferences.

• Cleans and maintains therapy equipment ensuring that equipment is in safe and proper working order.

• Assists with audits as requested.

• Consults with therapist, nurses, social workers, and other health care workers; contributing to patient care conferences.

• Assures continuation of therapeutic plan following discharge by designing and instructing patients, families, and caregivers in home exercise programs; recommending and/or providing assistive equipment; recommending outpatient or home health follow-up programs.

• Documents patient care services by charting in patient and department records.

• Maintains patient confidence and protects hospital operations by keeping information confidential.

• Maintains safe and clean working environment by complying with procedures, rules, and regulations.

• Protects patients and employees by adhering to infection-control policies and protocols.

• Ensures operation of equipment by completing preventive maintenance requirements; following manufacturer's instructions; troubleshooting malfunctions; calling for repairs.

• Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies.-Develops occupational therapy staff by providing information; developing and conducting in-service training programs.

• Complies with federal, state, and local legal and certification requirements by studying existing and new legislation; anticipating future legislation; enforcing adherence to requirements; advising management on needed actions.

• Contributes to team effort by accomplishing related results as needed.

Qualifications 

• Degree from an accredited Physical Therapist Assistant Program (approved by the APTA) 

• A minimum of six months physical therapist assistant experience preferred 

• Home Health experience a plus 

• Current and unrestricted PTA licensure 

• Current CPR certification 

• Good organizational and communication skills

Physical therapist assistants (PTAs) work as part of a team to provide physical therapy services under the direction and supervision of the physical therapist. PTAs implement selected components of patient/client interventions (treatment), obtain data related to the interventions provided, and make modifications in selected interventions either to progress the patient/client as directed by the physical therapist or to ensure patient/client safety and comfort.

PTAs assist the physical therapist in the treatment of individuals of all ages, from newborns to the     very oldest, who have medical problems or other health-related conditions that limit their abilities to move and perform functional activities in their daily lives.

The physical therapist is responsible for the services provided by the PTA. Physical therapists (PTs) are health care professionals who examine each individual and develop a plan using treatment techniques to promote the ability to move, reduce pain, restore function, and prevent disability. In addition, PTs work with individuals to prevent the loss of mobility before it occurs by developing fitness- and wellness-oriented programs for healthier and more active lifestyles.

PTAs provide care for people in a variety of settings, including hospitals, private practices, outpatient clinics, home health agencies, schools, sports and fitness facilities, work settings, and nursing homes. PTAs must graduate from a CAPTE-accredited PTA program and licensure or certification is required in most states in which a PTA works.
