Description

This position will act as a primary case manager/patient support specialist for the FEP program. The position will actively coordinate patient cases, assess and determine needs and support clinicians serving the patient. The case manager will track both patient and family progress through the program. Incumbent will work closely with the referral position and the centralized assessor as well as other clinicians. The incumbent will also serve as the outreach specialist for program patient needs. This individual will participate as a member of the Coordinated Specialty Care team and therefore be required to attend the National NAVIGATE Training and weekly team meetings. Requirements are to identify appropriate services for existing patients and those not accepted; participate in the team meetings and treatment plan; assessment of overall services required; coordinate with insurance; after hours calls; establishing community relationships for robust outbound service options.

Full time at 30 hours a week, hourly rate, eligible for overtime and benefits. 

 

 
Position Requirements
  • EDUCATION AND EXPERIENCE: Bachelor's degree from an accredited college or university in social work, psychology, or closely related human services field and one year of experience assisting professionals in providing direct client care and implementing treatment and/or service/care coordination plans for individuals with mental illness or developmental disabilities; OR an equivalent combination of experience and education above the bachelor's degree level.
    ENTRY LEVEL KNOWLEDGE, SKILLS, AND ABILITIES (required at time of application): General knowledge of: issues of daily living as applied to individual and family centered environments; therapeutic interpersonal interactions both verbal and non-verbal; substance abuse problems, organic problems and other conditions which may appear similar to a mental illness or developmental disability; symptoms and characteristics of mental illnesses and developmental disabilities; common clinical intervention practices; human growth and development.
    Ability to: make mental and behavioral assessments and accurately record information; prepare service/care coordination plans and provide services for a case load of clients; evaluate and reassess the functioning level of the client; provide instruction and direction to clients and others in group or individual settings; write clear and concise reports related to client progress and status; read and follow oral and written instructions, policies and procedures; defuse aggressive and maladaptive behaviors of clients; research and compile instructional material appropriate to teach daily living skills to clients; complete billing forms and documentation related to services provided.

  • Travel locally 50% of their time dependent on client and referral needs. </li>
  • 10% of travel will require transport of clients (ie to and from appointments; supplementary support such as finding housing, etc.)</li>
  • Requires Nevada driver's license in good standing, full auto insurance, and vehicle</li>
  • This position will be expected to travel at least once per year for training that lasts a period of 3 days. This training alternates between the North (Reno/Carson area) and South (Las Vegas). Individuals may also choose to travel to supplementary training in other parts of the country, but is not required. Final Note: Travel is currently suspended with training conducted via Zoom due to COVID-19 restrictions.</li></ul>
