Summary of responsibilities include: Serves as an active contributor in designated center meetings Complete psychosocial assessment with patient and family  Ensures patient and residents discharge goals are identified Works with patient and families to provide care planning Collaborate with interdisciplinary team Charts resident assessment and MDS, progress notes, and additional records/DMS     Position Requirements Requires: BSW required, MSW preferred License Required LBSW required. LMSW, LCSW, or LCSW-C preferred Long-term care, MDS (PHQ9 and BIMS), MOLST, Care Plans, Discharge planning, and NOMNCs experience preferred.
