Responsible for aftercare planning and aftercare Groups for all patients assigned to their caseload. Essential Job Duties and Responsibilities Meet in a timely manner with each patient to develop an aftercare plan. Conduct aftercare groups focusing on identifying aftercare needs, relapse and aftercare planning options. Provide family services when needed through family counseling and/or educational group modalities. Participate fully in Team Treatment meetings, Unit meetings, and Program meetings. Assist patients in maintaining contact with external systems where necessary in regard to Probation or Parole Officer, DPA, DHS, employer's funding resources, etc. Maintain all appropriate documentation and correspondence relative to each patient's individual medical record, according to Hospital, State and Federal Standards. Assists clients in maintaining contact with external systems when necessary including: Probation or Parole Officer, DPA, DHS, Employers, Funding Sources, and other case management as required. Completes Informed Consents for each contact person/agency as needed and documents properly. Consults with other staff members to ensure quality patient care.   Position Requirements BSW with work experience related to the field of substance dependence treatment is desirable.   This position is currently not accepting applications.
